Repercussions of violence on the mental health of workers of the Family Health Program

Resumo: | OBJECTIVE: To describe forms of external and indirect violence that affect the mental health of workers of the Programa Saúde da Família (Family Health Program), as well as the strategies developed by these workers to enable their work and to be psychologically protected. METHODS:Qualitative study on the Programa Saúde da Família work process, performed in the cities of São Paulo, Ribeirão Preto and Embu (Southeastern Brazil) in 2005. Theoretical approach of psychodynamics of work, which proposes the formation of reflection groups with workers, was employed. Subjective aspects of work, situations of psychological suffering and strategies used by workers to deal with suffering and continue to work were sought to be identified. RESULTS:The Program's work organization exposed workers to the following: situations of violence, invisible at times; feeling of impotence in the face of precarious situations; lack of acknowledgement of efforts made; lack of borders between professional and personal aspects; intense experiences of social and domestic violence; fear of risk of exposure; feeling of moral and physical integrity being threatened; and fear of reprisal. Situations of psychological suffering resulting from violence in the workplace were observed. These became more intense in the Programa Saúde da Família due to regular contact with situations of violence that cause fear and a feeling of vulnerability. CONCLUSIONS:Psychological repercussions caused by violence in the workplace, not always expressed in the form of psychological disorders, were observed in situations of intense suffering. Workers develop strategies to minimize suffering, protect themselves psychologically and continue to work; and seek to create solidarity and protection networks with the population, aiming to reduce vulnerability. With the experience gained, they learn to detect high-risk situations, avoiding those they believe to be threatening. |

OBJETIVO: Describir formas de violencia externa e indirecta que afectan la salud mental de trabajadores de programa de salud de la familia, bien como estrategias desarrolladas por los trabajadores para viabilizar su trabajo y protegerse psicológicamente. MÉTODOS: Estudio cualitativo del proceso de trabajo en el Programa Salud de la Familia, realizado en los municipios de Sao Paulo, Ribeirao Preto y Embu (Sureste de Brasil), en 2005. Fue utilizado el abordaje teórico de la psicodinámica del trabajo, que propone la formación de grupos de reflexión con los trabajadores. Se buscó identificar aspectos subjetivos del trabajo, situaciones de sufrimiento psíquico y estrategias utilizadas por los trabajadores para lidiar con el sufrimiento y continuar trabajando. RESULTADOS:La organización del trabajo en el Programa expone a los trabajadores a: situaciones de violencia, muchas veces invisible; sentimientos de impotencia frente a situaciones de precariedad; no reconocimiento de esfuerzos realizados; falta de fronteras entre aspectos profesionales y personales; convivencia intensa con situaciones de violencia doméstica y social; miedo del riesgo de exposición; sensación de integridad moral y física amenazadas y temor de represalia. Fueron observadas situaciones de sufrimiento psíquico producto de la violencia en el trabajo, intensificados en el Programa Salud de la Familia por la convivencia cotidiana con situaciones de violencia que generan miedo y sentimiento de vulnerabilidad. CONCLUSIONES: Las repercusiones psicológicas generadas por la violencia en el trabajo, no siempre expresadas bajo la forma de trastornos psíquicos, fueron observadas en situaciones de elevado sufrimiento. Los trabajadores desarrollan estrategias para minimizar ese sufrimiento, se protegen psíquicamente y continúan trabajando; buscan construir redes de solidaridad y de protección con la población visando la disminución de la vulnerabilidad. Aprenden, en la experiencia acumulada, a detectar situaciones de riesgo evitando aquellas que consideran amenazantes. Foram observadas situações de sofrimento psíquico decorrente da violência no trabalho, intensificados no Programa Saúde da Família pelo convívio cotidiano com situações de violência que geram medo e sentimento de vulnerabilidade. CONCLUSÕES: As repercussões psicológicas geradas pela violência no trabalho, nem sempre expressas sob a forma de transtornos psíquicos, foram observadas em situações de elevado sofrimento. Os trabalhadores desenvolvem estratégias para minimizar esse sofrimento, se protegem psiquicamente e continuam a trabalhar; buscam construir redes de solidariedade e de proteção com a população visando à diminuição da vulnerabilidade. Aprendem, na experiência acumulada, a detectar situações de risco evitando aquelas que acreditam serem ameaçadoras. Universidade de São Paulo.
