An epidemiologic study of silicosis in the Southeast Region of Brazil, from a survey of patients in tuberculosis hospitals

Resumo: To increase current knowledge concerning the epidemiology of silicosis in the southeast of Brazil, a detailed survey was carried out through the occupational histories of 3,440 adult male patients admitted for tuberculosis to 27 tuberculosis hospitals in the states of S. Paulo, Rio de Janeiro, and Minas Gerais (Brazil). Analysis of complete occupational histories disclosed 327 (9.5%) suspected cases of silicotuberculosis. Random and independent readings of chest roentgenograms by three experienced professionals (two pneumologists and one radiologist) confirmed positive or suspected shadows of pneumoconiosis in 119 patients (3.5%). Applying this rate of silicotuberculosis to the total number of adult male admissions in 1977 and the tuberculosis frequency in silicotics in Brazil - 3.3% - indicates an estimated 20,000 cases of silicotics in southeastern Brazil. The 119 patients with confirmed or suspected shadows furnished epidemiological characteristics, such as, age, occupation, and length of exposure, which are presented. The importance is stressed of silicosis as a public health problem in this region and as an occupational disease meriting priority by the occupational health agencies authorities in Brazil.

Com base no conhecimento de que a tuberculose pulmonar é freqüente complicação evolutiva da silicose (caracterizando a sílicotuberculose), foi realizado estudo visando a ampliar o conhecimento da epidemiologia desta doença profissional na Região Sudeste do Brasil, através do levantamento detalhado dos antecedentes ocupacionais de 3.440 pacientes masculinos, adultos, internados por tuberculose, em 27 Hospitais de Tisiologia, nos Estados de São Paulo, Rio de Janeiro e Minas Gerais (Brasil). Através da anamnese ocupacional foram detectados 327 "prováveis casos" de sílicotuberculose (9,5%). As leituras independentes e "cegas" das radiografias de tórax, feitas por três profissionais experientes em silicose (2 Pneumologistas e 1 Radiologista), confirmaram a presença de imagens positivas ou suspeitas de pneumoconiose, em 119 pacientes (3,5%). Esta proporção de casos de sílicotuberculose, aplicada ao número total de internações de adultos masculinos, ocorridas em 1977, e à freqüência da tuberculose em pacientes silicóticos, de acordo com a experiência brasileira (3,3%), permitiu estimar em 20.000 o número aproximado de portadores de silicose na Região Sudeste do Brasil. Idêntico raciocínio permitiu estimar em 5.100 o número de portadores de silicose no Estado de São Paulo, 6.900 no Rio de Janeiro e 7.400 em Minas Gerais. Com base na amostra de 119 casos de sílicotuberculose encontrados, são estudados alguns caracteres epidemiológicos da silicose nessa Região: idade dos pacientes, atividades ocupacionais exercidas e tempo de exposição. Concluiu-se pela importância da silicose como "problema de saúde pública" nessa Região, e como "doença profissional" que deveria merecer prioridade dos órgãos responsáveis pela Saúde Ocupacional no Brasil.
