Time trends and factors associated with breastfeeding in the state of Pernambuco, Northeastern Brazil

Resumo: OBJECTIVE: To analyze time trends in the duration of exclusive breastfeeding and associated protection factors. METHODS: Prevalances for total breastfeeding and exclusive breastfeeding in a population of four-month-old infants were analyzed. Data were obtained from population surveys carried out in 1991, 1997, and 2006, with 935, 2081, and 1568 infants respectively. The data were retrieved from interviews answered by parents or guardians. The prevalences were analyzed by Poisson regression in regard to: environmental, behavioral and socioeconomic conditions, maternal variables, and biological factors of infant. RESULTS: The median duration of total breastfeeding rose from 89 days (1991) to 106 days (1997), and finally to 183 days (2006). The median duration of exclusive breastfeeding remained around 30 days between 1997 and 2006. In the multivariate analysis, from the ten variables studied, only maternal schooling and age, address, and female gender of infant were maintained in the final explanatory model. CONCLUSIONS: Despite the significant increase in terms of total breastfeeding duration, the same did not occur with the duration of exclusive breastfeeding.

MÉTODOS: Fueron analizadas las prevalencias de amamantamiento total y el amamantamiento materno exclusivo de lactantes a los cuatro meses de edad. Los datos fueron obtenidos de pesquisas poblacionales realizadas en 1991, 1997 y 2006, con 935, 2.081 y 1.568 niños, respectivamente. Los datos fueron colectados por entrevista realizada con el responsable del niño. Las prevalencias fueron analizadas por regresión de Poisson en relación a: condiciones ambientales, de comportamiento y socioeconómicas, variables maternas y factores biológicos del niño. RESULTADOS: La duración mediana del amamantamiento total se elevó de 89 días (1991) a 106 días (1997) y, finalmente, 183 días (2006). La mediana del amamantamiento materno exclusivo se mantuvo estacionario en torno de 30 días entre 1997 y 2006. En el análisis multivariado, de las diez variables analizadas, sólo la escolaridad y las edades maternas, situación del domicilio y el sexo femenino del niño se mantuvieron en el modelo explicativo final. CONCLUSIONES: A pesar de que el amamantamiento total tuvo aumento importante de duración, no ocurrió lo mismo con el amamantamiento materno exclusivo. MÉTODOS: Foram analisadas as prevalências de amamentação total e o aleitamento materno exclusivo de lactentes aos quatro meses de idade. Os dados foram obtidos de inquéritos populacionais realizados em 1991, 1997 e 2006, com 935, 2.081 e 1.568 crianças, respectivamente. Os dados foram coletados por entrevista realizada com o responsável pela criança. As prevalências foram analisadas por regressão de Poisson em relação a: condições ambientais, comportamentais e socioeconômicas, variáveis maternas e fatores biológicos da criança. RESULTADOS: A duração mediana do aleitamento total elevou-se de 89 dias (1991) para 106 dias (1997) e, finalmente, 183 dias (2006). A mediana do aleitamento materno exclusivo manteve-se estacionária em torno de 30 dias entre 1997 e 2006. Na análise multivariada, das dez variáveis analisadas, apenas a escolaridade e idade maternas, situação do domicílio e a criança ser do sexo feminino se mantiveram no modelo explicativo final. CONCLUSÕES: Apesar do aleitamento total ter tido importante aumento de duração, o mesmo não ocorreu com a duração do aleitamento materno exclusivo. Universidade de São Paulo.
