Diagnostic laparoscopy in dogs: analysis of 27 cases

Resumo: Laparoscopy was used for the visualization of abdominal viscera of 27 dogs afflicted with different pathologies. In all cases general anesthesia was applied. An incision was made on the ventral midline approximately 1cm caudal to the navel. At this point a trocar was introduced for the production of pneumoperitoneum. After that, the optical probe was introduced for inspection of the abdominal cavity. The different organs were visually identified by their external morphology. Moreover, the exam also allowed the identification of different pathologies. In two cases of splenic and pancreatic atrophy, as well as in case of hepatic necrosis the laparoscopic examination was instrumental for the understanding of the disease, leading to a final diagnostic confirmed by histopathological analysis. The presence of hepato-frenic adhesion was immediately identified in one case, helping in the planning of corrective surgery. Cases of piometritis in initial stages were identified thanks to the laparoscopic exam, since biochemical and radiological data was highly inconclusive. The laparoscopy determined the impossibility of surgical removal of two hepatic tumors and one case of intestinal neoplasia. The results most likely influenced the request for euthanasia by the owners. The present observations agree with citations in which laparoscopy cannot avoid major surgery, however it gives valuable information regarding the conditions of the tissues and the extension of the lesions.

Foi empregada anestesia geral, sendo o exame realizado via linha mediana ventral, 1 cm caudalmente à cicatriz umbilical. Neste ponto, foi introduzido o trocarte para instalação do pneumoperitônio e posteriormente inserida a óptica para a inspeção intracavitária. Os órgãos foram reconhecidos por suas características morfológicas externas, sendo possível a identificação de diferentes afecções. Em dois casos de atrofia esplênica e pancreática, bem como num caso de necrose hepática, o exame laparoscópico permitiu melhor esclarecimento da patologia existente, conduzindo a suspeita clínica ao diagnóstico final confirmado pela análise histopatológica. A presença de aderência hepatofrênica em um animal foi imediatamente diagnosticada, fornecendo fundamental informação ao desenvolvimento do ato operatório posterior. Os casos de tumoração e de piometrite em fase inicial foram definitivamente estabelecidos graças ao exame laparoscópico, já que os exames laboratoriais e radiológicos foram inconclusivos, corroborando várias citações de literatura segundo as quais a laparoscopia é decisiva nas afecções não estabelecidas satisfatoriamente pelos métodos convencionais. A laparoscopia determinou a inoperabilidade de tumorações hepáticas em dois casos e intestinal em um caso, influindo na decisão de solicitação de eutanásia dos animais pelos proprietários. As observações obtidas na casuística examinada levam à concordância com a citação de que a laparoscopia pode não evitar uma cirurgia de maior envergadura, porém fornece valiosas informações como o aspecto tecidual e extensão da lesão, indicando a definitiva manipulação.Universidade de São Paulo.
