Efficacy of two muscle stretching modalities in treating the patellofemoral syndrome: a comparative study

Resumo: | The aim of this study was to compare the efficacy of muscle stretching exercises on the functional recovery of patients with patellofemoralsyndrome. Twenty female young nonathletes with PFS were divided into two groups: group 1 (G1) performed posterior chain global stretching exercises and group 2 (G2), segment stretching exercises of hamstring and gastrocnemii muscles. The following were assessed, before and after treatment: pain intensity, by the visual analog scale; knee functional level; flexibility, by the fingertip-to-floor test; hamstring shortening; Q angle; and electromyography of the biceps femoris and lateral gastrocnemius muscles. Treatment consisted of two weekly sessions during eight weeks. Data obtained for each group and between groups were statistically analysed, with significance level set at p<0.05. Results show significant improvement in knee functional level, hamstring shortening, Q angle, and flexibility in both groups; but only G1 showed decreased pain intensity; compared to G2, G1 showed greater flexibility gain. These results suggest that muscle stretching exercises, especially global stretching, may be prescribed in PFS treatment for effective pain reduction. Moreover, the treatment allowed for better knee alignment and flexibility gain, which may favour muscle strengthening. |

Foram selecionadas 20 mulheres jovens sedentárias com SFP, divididas em dois grupos: o grupo 1 (G1) realizou alongamento dos músculos da cadeia posterior pela técnica de reeducação postural global e o grupo 2 (G2) alongamento segmentar dos músculos isquiotibiais e gastrocnêmio. Foram avaliadas: a intensidade da dor no joelho, pela escala visual analógica; capacidade funcional; flexibilidade pelo teste 3o dedosolo;encurtamento dos músculos isquiotibiais; ângulo Q; e eletromiografia dosmúsculos bíceps femoral e gastrocnêmio porção lateral. O tratamento, com duas sessões semanais, durou oito semanas. Os dados colhidos antes e após o tratamento em cada grupo e entre os grupos foram analisados estatisticamente, com nível de significância fixado em p<0,05. Após o tratamento, os resultados mostram que os dois grupos obtiveram melhora em capacidade funcional, encurtamento dos músculos isquiotibiais, ângulo Q e flexibilidade; porém, só o G1 relatou redução na intensidade da dor. Comparado ao G2, o G1 teve melhor índice de flexibilidade. Os resultados sugerem que os exercícios de alongamento muscular, em especial o global, devem ser indicados no tratamento de pacientes com SFP, para uma redução efetiva da dor. O tratamento também possibilitou melhor realinhamento dos joelhos e aumento daflexibilidade, o que pode facilitar o fortalecimento muscular.Universidade de São Paulo.
