Mandibular second premolars distoangulation: a longitudinal follow-up from the mixed to the permanent dentition

Resumo: the mixed to the permanent dentition Objective: The aim of this study was to assess the angular changes of mandibular second premolars (MnP2) with distoangulation and to evaluate the different clinical outcomes after a longitudinal follow-up. Methods: The sample was collected from the orthodontic records of 865 patients who sought phase I treatment at one single center. Distoangulation group was composed by 42 patients (mean age 9.29±1.24 years, 16 male, 26 female) with distoangulation of MnP2. The control group was composed by 32 patients (mean age 9.38±1.10 years, 15 male, 17 female) without distoangulation of MnP2. Panoramic radiographs taken in the mixed (T1) and the early permanent dentition (T2) were analyzed in both groups. Longitudinal angular changes, the degree of root formation, second premolar depth and the prevalence of associated dental anomalies were analyzed. Intergroup comparison was performed with Mann-Whitney and t tests. Intergroup comparison of dental anomaly frequency was performed using Chi-square tests (p<0.05). Results: The prevalence of MnP2 distoangulation in the mixed dentition was 4.85%. The distoangulation group showed a greater initial angle (59.34o±8.41) when compared to the control group (79.88o±7.60). The spontaneous eruption of the MnP2 with distoangulation was observed in 76.57% of the sample. The extraction of the predecessor second deciduous molar followed by the spontaneous eruption was found in 23.43%. MnP2 distoangulation was significantly associated with agenesis of its antimere, small maxillary lateral incisors and deciduous molar infraocclusion. Conclusions: Conservative approaches may be preferred in cases of mandibular second premolar distoangulation. Severe cases of MnP2 distoangulation were rare.

Severe cases of MnP2 distoangulation were rare.Objetivo: O objetivo deste estudo foi avaliar as alterações angulares dos segundos pré-molares inferiores (2°PMI) com distoangulação e avaliar os diferentes resultados clínicos após um acompanhamento longitudinal. Material e métodos: A amostra foi coletada dos prontuários ortodônticos de 865 pacientes que procuraram por tratamento ortodôntico preventivo em um único centro. O grupo distoangulação foi composto por 42 pacientes (média de idade 9,29±1,24 anos, 16 homens, 26 mulheres) com distoangulação do 2°PMI. O grupo controle foi composto por 32 pacientes (média de idade 9,38±1,10 anos, 15 homens, 17 mulheres) sem distoangulação do 2°PMI. Radiografias panorâmicas tiradas na dentadura mista (T1) e permanente jovem (T2) foram analisadas em ambos os grupos. Alterações angulares longitudinais, o grau de formação da raiz, a profundidade do segundo pré-molar e a prevalência de anomalias dentárias associadas foram analisadas. A comparação intergrupos foi realizada com Mann-Whitney e Testes t. A comparação intergrupos da frequência de anomalias dentárias foi realizada por meio do teste qui-quadrado (p<0,05). Resultados: A prevalência da distoangulação do 2°PMI na dentadura mista foi de 4,85%. O grupo distoangulação apresentou maior ângulo inicial (59,34°±8,41) quando comparado ao grupo controle (79,88°±7,60). A erupção espontânea do 2°PMI com distoangulação foi observada em 76,57% da amostra. A exodontia dos dentes decíduos predecessores seguida da erupção espontânea do 2°PMI foi encontrada em 23,43%. A distoangulação do 2°PMI foi significativamente associada com agenesia de seu antímero, microdontia de incisivos laterais superiores e infraoclusão de molares decíduos. Conclusão: Abordagens conservadoras podem ser preferidas em casos de distoangulação do segundo pré-molar inferior.
