Assistência domiciliar prestada por equipes estratégia saúde da família a idosos com doenças crônico-degenerativas em um município do interior de São Paulo

One of the greatest difficulties concerning the aging process, both for the elderly and for the respective family, is facing a chronic degenerative disease (CDD), considering that expectations in these cases are of a constant worsening of the clinical condition. Considering that increasing numbers of elderly individuals will face CDD, reflecting on how public health in Brazil is organized to assist theses individuals in their communities is a necessity. Considering this scenario, the present study aimed to analyze the home care provided for elderly CDD patients by ESF teams in a municipality in inner São Paulo, since the ESF proposal is to facilitate integral access and attendance to all families in the communities, guaranteeing health action continuity, longitudinality and resolutiveness. This is a qualitative study, carried out through participant observation and focal groups comprising six members belonging to two ESF teams in the city, one serving the urban and the other, the rural, population. The results were submitted to a content analysis. The results indicate the need for service reorganization, in order to strengthen the population assistance network and point out the need for a service reorganization, in order to strengthen the network to provide assistance to the population. The reports of the participating professionals indicate lack of structuring concerning attendance, locomotion difficulties, lack of professionals and lack of collaboration of family members in the monitoring and treatment of specific scenarios. These complaints present significant difficulties in the execution of team assignments, delaying care, providing precarious home care. The research indicates that several challenges exist in this municipality in order to consolidate the primary care public policy. It is understood that the elaboration of therapeutic projects created and developed by multiprofessional and intersectorial teams would be the main outlet for the search of solutions.
