Tratamento conservador em lesão intraóssea mandibular: relato de caso clínico

Resumo: Diagnosis is a fundamental step, which intends to exclude the disease and rehabilitate the patient. Complementary, imaging (2D, 3D) and laboratory tests are essential for a correct diagnosis, follow-up, proservation, and conduct of the professional for appropriate treatment. This paper reports a clinical case of mandibular intraosseous lesion in an elderly, systemically compromised patient, focusing on the diagnostic construction steps. The diagnostic hypotheses of this case were: Ameloblastoma, Central Lesion of Giant Cells and Odontogenic Keratocyst. These lesions are recurrent and infiltrative, and may reach large and generally asymptomatic lesions, such as the lesion described in this clinical case. Patient M.O.S, 63 years old, male gender, was attended at the Dentistry Clinic of the Southern University of Santa Catarina for an evaluation. In the extra-oral clinical examination, he was within the normal range, but when the intra-oral evaluation was made, an increase in volume was observed in the buccal mucosa at the bottom of the sulcus in the lower right region. After the periapical radiographic examination of the region, a well-defined unilocular radiolucent area was observed in the lower canine region of the right lower first molar. Due to the extension of the lesion, a panoramic radiograph was requested to better visualize the lesion. According to the diagnostic hypotheses reported in the radiographic report, a cone beam computed tomography was indicated for better diagnostic definition and anatomical involvement. As the patient presented a delicate systemic condition for the procedure of total removal of the lesion to be performed in the Dentistry clinic, a minimally invasive surgery, the incisional biopsy, was performed in order to obtain the definitive diagnosis. The puncture / aspiration was also used, and then it was anchored in the marsupialization technique, aiming at reduction the lesion, a fact demonstrated in the panoramic radiographic image after 7 months after the intervention.

Exames complementares, de imagem (2D, 3D) e laboratoriais são imprescindíveis para um correto diagnóstico, acompanhamento, proservação, e conduta do profissional para adequado tratamento. Este trabalho relata o caso clínico de lesão intraóssea mandibular em um paciente idoso, comprometido sistemicamente, enfocando as etapas de construção do diagnóstico. As hipóteses diagnósticas deste caso, foram: Ameloblastoma, Lesão Central de Células Gigantes e Ceratocisto Odontogênico. Essas lesões são de caráter recidivante e infiltrativas, podendo atingir grandes dimensões e geralmente assintomáticas, como a lesão descrita neste caso clínico. Paciente M.O.S, 63 anos de idade, gênero masculino, foi atendido na Clínica de Odontologia da Universidade do Sul de Santa Catarina para uma avaliação. No exame clínico extra bucal apresentava-se dentro dos padrões de normalidade, mas quando realizado a avaliação intrabucal, notou-se um aumento de volume na mucosa bucal, fundo de sulco, inferior direita. Realizado o exame radiográfico periapical da região, observou-se área radiolúcida, unilocular, bem definida, na região de canino inferior direito à primeiro molar inferior direito. Devido à extensão da lesão, solicitou-se uma radiografia panorâmica para melhor visualização da lesão. De acordo com as hipóteses diagnósticas relatadas no laudo radiográfico, indicou-se a realização de tomografia computadorizada cone beam, para melhor definição diagnóstica e do envolvimento anatômico. Como o paciente apresentava uma condição sistêmica delicada para o procedimento de remoção total da lesão ser realizado na clínica de Odontologia, primeiramente procedeu-se uma cirurgia minimamente invasiva, a biópsia incisional, a fim de obter o diagnóstico definitivo.
