Regeneração óssea guiada com uso de barreira de membrana d-PTFE (denso) e osso xenógeno bovino particulado versus coágulo sanguíneo: avaliação clínica e tomográfica

Resumo: Introduction/Goal: Examine the performance of new materials for bone reconstruction and related areas within dentistry, based on clinical and tomographic analysis. Failure to perform procedures to preserve the alveolus can cause aesthetic damage, not maintaining the contour of the border, causing reabsorption and bone unevenness in the arch, leading to complicated follow-up treatments such as lack of structures for posterior implantation and a space between the collar and the dental prosthesis. Methodology: Three patients with a total of 12 teeth were selected for extraction. The method used was split-mouth, which consists of using the two experiments in the same patient. In the GT group the alveoli after extraction were filled with Lumina Bone-Porous bone graft material (CRITERIA®) and later covered with a PTFE membrane (CRITERIA®), in the GC group the alveoli were filled by the clot itself to be formed and subsequently covered with the PTFE membrane (CRITERIA®). Results: They showed minimal difference between the groups. The presence of the membrane provides a lower bone resorption and a greater gain in both group. In the tomographic analysis in the variables of the dependent group, the vestibular board had a significant gain of 4, 56 mm in the control group and in the variable height of the alveolus the gain was 1, 69 mm in the test group, in the other variables in both groups during the Three was minimal reabsorption where the loss was ≤ 1, 5 mm. In the clinical analysis evaluation in millimeters of thickness and height, both groups had gains, much as a result of the use of PTFE membrane in both, where the increase of keratinized gingiva was 5, 00 mm in the test group 4, 90 mm in the group control. Conclusion: The permanence of the PTFE membrane in position for 21 days is of vital importance for the success of the treatment.

A não realização de procedimentos de preservação do alvéolo pode originar prejuízo estético, não mantendo o contorno do rebordo, causando reabsorção e desnível ósseo na arcada, levando a tratamentos de acompanhamento complicados como a falta de estrutura para posterior implante e um espaço entre o rebordo e a prótese dentomucossuportada. Metodologia: Foram selecionados 3 paciente com um total de 12 dentes indicados para extração. O método utilizado foi split-mouth, que consiste em utilizar os dois experimentos no mesmo paciente. No grupo GT os alvéolos após a extração foram preenchidos com material de enxertia óssea Lumina Bone-Porous (CRITÉRIA®) e recoberto com uma membrana de PTFE (CRITÉRIA®), no grupo GC os alvéolos foram preenchidos pelo próprio coágulo e recoberto com a membrana de PTFE (CRITÉRIA®). Resultados: Houve mínima diferença entre os grupos. A presença da membrana proporcionou uma menor reabsorção óssea e um maior ganho em ambos os grupos. Na análise tomográfica nas variáveis do grupo dependente, a tábua vestibular teve um ganho significativo de 4,56 mm no grupo controle e na variável altura do alvéolo o ganho foi de 1,69 mm no grupo teste, nas outras variáveis em ambos os grupos durante o curso da cicatrização houve uma reabsorção mínima onde a perda foi ≤ a 1,5 mm. Na análise clínica avaliação em milímetros de espessura e altura, os dois grupos obtiveram ganhos, muito por consequência do uso da membrana de PTFE em ambos, onde o aumento de gengiva ceratinizada foi de 5,00 mm no grupo teste 4,66 mm no grupo controle.
