Psychological violence and the role of the psychiatrist: a narrative review

Resumo: Introduction: Psychological violence is a form of domestic violence characterized by verbal aggression, embarrassment, humiliation, manipulation, threats and isolation of the victim. Despite being the most prevalent form of intimate partner violence, psychological violence is the most underreported and can evolve into more serious aggressions, such as physical and sexual abuse, when there is no external intervention. Methods: Narrative review aimed at discussing the state of the art and updating knowledge about psychological violence against women. Discussion: The psychological damage caused by this violence can lead to depression, anxiety, post-traumatic stress, substance abuse and suicidal ideation, which can be identified by the psychiatrist in clinical practice. The psychiatrist can also act in the pharmacological and psychotherapeutic treatment of the consequences of psychological abuse in the victims, in addition to offering support and guidance in eventual denunciation and/or separation processes. Due to the high comorbidity with substance use disorder in the aggressors, the psychiatrist can act in treatment rehabilitation programs, preventing the occurrence of new aggressions. In the forensic setting, the psychiatrist can conduct expertise and technical assistance to document the damage suffered by the victims, in addition to assessing the risk of violence and criminal liability of the aggressors. Conclusion: Psychological violence is a public health problem with serious consequences. As a health professional, the psychiatrist has the potential to act in several areas to combat domestic violence.

A pesar de ser la forma más prevalente de violencia de pareja, la violencia psicológica es la menos denunciada y puede evolucionar hacia agresiones más graves, como el abuso físico y sexual, cuando no hay intervención externa. Métodos: Revisión narrativa con el objetivo de discutir el estado del arte y actualizar el conocimiento sobre la violencia psicológica contra la mujer. Discusión: El daño psicológico causado por esta violencia puede derivar en depresión, ansiedad, estrés postraumático, abuso de sustancias e ideación suicida, los cuales pueden ser identificados por el psiquiatra en la práctica clínica. El psiquiatra también puede actuar en el tratamiento farmacológico y psicoterapéutico de las consecuencias del maltrato psicológico en las víctimas, además de ofrecer apoyo y orientación en eventuales procesos de denuncia y/o separación. Debido a la alta comorbilidad con el trastorno por consumo de sustancias en los agresores, el psiquiatra puede actuar en los programas de rehabilitación del tratamiento, previniendo la aparición de nuevas agresiones. En el ámbito forense, el psiquiatra puede realizar peritajes y asistencia técnica para documentar el daño sufrido por las víctimas, además de evaluar el riesgo de violencia y la responsabilidad penal de los agresores. Conclusión: La violencia psicológica es un problema de salud pública con graves consecuencias. Como profesional de la salud, el psiquiatra tiene el potencial de actuar en varias áreas para combatir la violencia doméstica.Introdução: A violência psicológica é uma forma de violência doméstica caracterizada por agressões verbais, constrangimento, humilhação, manipulação, ameaças e isolamento da vítima. Apesar de ser a forma mais prevalente de violência por parceiro íntimo, a violência psicológica é a mais subnotificada, podendo evoluir para agressões mais graves, como abusos físicos e sexuais, quando não há intervenção externa. Métodos: Revisão narrativa visando discutir o estado da arte e atualização do conhecimento sobre violência psicológica contra a mulher. Discussão: O dano psicológico causado por esta violência pode levar a quadros de depressão, ansiedade, estresse pós-traumático, abuso de substâncias e ideação suicida, que podem ser identificados pelo psiquiatra na prática clínica. O psiquiatra também pode atuar no tratamento farmacológico e psicoterápico das consequências do abuso psicológico nas vítimas, além de oferecer apoio e orientações em eventuais processos de denúncia e/ou separação. Pela alta comorbidade com transtorno por uso de substâncias nos agressores, o psiquiatra pode atuar em programas de reabilitação de tratamento, prevenindo a ocorrência de novas agressões. No setting forense, o psiquiatra pode conduzir perícias e assistências técnicas para documentar os danos sofridos pelas vítimas, além de avaliar risco de violência e imputabilidade penal dos agressores.
