Social determinants of disordered eating behaviors in Brazilian adolescents

Resumo: Introduction: The increasing occurrence of eating disorders among adolescents requires the identification of disordered eating behaviors (DEB) and social determinants related to these health problems. Objective: This study aimed to assess the prevalence of DEB among Brazilian school-age children and their associations with social determinants of health. Method: 16,556 adolescents from the 2015 PeNSE (a Brazilian National School-based Student Health Survey) were evaluated. Poisson regressions estimated multivariate models related to inadequate compensatory methods for weight control (laxatives/purgatives or medications/products). Results: DEB prevalence for weight loss in the group was 11.3% (95% CI: 10.6-12.0%) with associated factors of being male, non-white color/race, being 14 years old or younger, overweight, body image dissatisfaction, frequent consumption of fast food, working, not having regular meals with guardians, having mothers with lower education levels, households with five or more residents, experience of hunger at household, and studying in public schools. Conclusions: These DEB for weight loss were more prevalent among adolescents with greater body dissatisfaction and living in situations of higher social vulnerability.

Objetivo: Evaluar la prevalencia de trastornos alimentarios (EAC) entre escolares brasileños y sus asociaciones con los determinantes sociales de la salud. Metodología: Se evaluaron 16,556 adolescentes de la Encuesta Nacional de Salud Adolescente 2015. Las regresiones de Poisson estimaron modelos multivariados relacionados con métodos compensatorios inadecuados para el control de peso (laxantes / purgantes o medicamentos / productos). Resultados: La prevalencia de EAC para la pérdida de peso en el grupo fue del 11,3% (IC del 95%: 10,6-12,0%) con factores asociados: sexo masculino, color/raza no blanca, 14 años o menos, exceso de peso, insatisfacción. con imagen corporal, ir a comida rápida, trabajar, no comer regularmente con los tutores, tener madres con menos educación, vivir con cinco o más residentes, experiencia del hambre en el hogar y estudiar en escuelas públicas. Conclusiones: estos comportamientos desordenados para la pérdida de peso fueron más prevalentes entre los adolescentes con mayor insatisfacción corporal y que viven en situaciones de mayor vulnerabilidad social.Introdução: O crescimento de transtornos alimentares entre adolescentes demanda a identificação de comportamentos alimentares desordenados e de determinantes sociais relacionados a esses agravos. Objetivo: Avaliar as prevalências de comportamentos alimentares desordenados (CAD) entre adolescentes escolares brasileiros e suas associações com determinantes sociais em saúde. Metodologia: Foram avaliados 16.556 adolescentes da Pesquisa Nacional de Saúde do Escolar de 2015. Regressões de Poisson estimaram modelos multivariados relacionados a métodos compensatórios inadequados para controle de peso (laxativos/purgativos ou medicamentos/produtos). Resultados: A prevalência dos CAD para perda de peso no grupo foi 11,3% (IC95%:10,6-12,0%) tendo como fatores associados: sexo masculino, cor/raça não branca, 14 anos ou menos, excesso de peso, insatisfação com a imagem corporal, frequentar fast food, trabalhar, não realizar regularmente refeições com responsáveis, ter mães com menor escolaridade, domicílio com cinco ou mais moradores, vivência da fome no domicílio e estudar em escolas públicas.
