Epidemiological, clinical and therapeutic perspectives of bipolar disorder in comorbidity with drug use: literature review in portuguese

Resumo: Introduction: Bipolar disorder (BT) is a pathology that presents a high coefficient of comorbidity with other conditions in Mental Health, implying a considerable increase in complexity in clinical management. Objective: The objective of this research was to analyze the psychopathological condition of comorbidity between BT and drug use, paying attention to the epidemiological, clinical and therapeutic aspects in scientific articles in portuguese. Method: An integrative literature review was used as a methodological design. In the selection of articles, the following criteria were adopted time interval of 15 years (2005 to 2020), Portuguese language, typology of production articles and thematic area Health Sciences. Lilacs, Pepsic, Pubmed and Scielo databases were accessed. Result: Thirteen articles were obtained, which had their results analyzed and debated. It was observed that BD is a serious psychopathological condition with a high rate of comorbidities. The presence of a comorbid condition due to drug use intensifies the picture, as well as making therapeutic management difficult. It is pointed out that 60% to 85% of patients diagnosed with TB have already abusive used some drug during their lifetime. Among the most used by people with TB, alcohol represents the licit substance with the greatest association, affecting TB patients five times more than the general population. There was a scarcity of research on the subject in Portuguese. Conclusion: It is suggested the development of studies that point out specific clinical-therapeutic data of BD in comorbidity with different drugs, analyzing the effects of different substances on the symptomatology and therapeutic management.

Objetivo: El objetivo de esta investigación fue analizar la condición psicopatológica de comorbilidad entre TB y consumo de drogas, desde los aspectos epidemiológicos, clínicos y terapéuticos en artículos científicos en portugués. Método: Se utilizó como diseño metodológico una revisión integrativa de la literatura. En la selección de los manuscritos fueron adoptados los siguientes criterios: intervalo de tiempo de 15 años (2005 a 2020), lengua portuguesa, tipología de producción de artículos y área temática de Ciencias de la Salud.Se utilizaron las bases de datos Lilacs, Pepsic, Pubmed y Scielo. accedido. Resultado: Se obtuvieron trece artículos cuyos resultados fueron analizados y discutidos. Se observó que el TB es una condición psicopatológica grave con un alto índice de comorbilidades. La presencia de una comorbilidad por consumo de drogas intensifica el cuadro, además de dificultar el manejo terapéutico. Se señala que del 60% al 85% de los pacientes diagnosticados con TB ya han consumido algún fármaco en su vida. Entre los más utilizados por las personas con TB, el alcohol representa la sustancia lícita con mayor asociación, afectando a los pacientes con TB cinco veces más que a la población general. Hubo escasez de investigaciones sobre el tema en portugués. Conclusión: Se sugiere el desarrollo de estudios que señalen datos clínico-terapéuticos específicos del TB en comorbilidad con diferentes fármacos, analizando los efectos de diferentes sustancias sobre la sintomatología y el manejo terapéutico.Introdução: O transtorno bipolar (TB) é uma patologia que apresenta alto coeficiente de comorbidade com outros quadros em Saúde Mental, implicando considerável complexidade no manejo clínico. Objetivo: O objetivo desta pesquisa foi analisar a condição psicopatológica de comorbidade entre TB e uso de drogas a partir de aspectos epidemiológicos, clínicos e terapêuticos em artigos científicos em português. Método: Utilizou-se a revisão integrativa de literatura como delineamento metodológico. Na seleção dos manuscritos, adotou-se como critérios: intervalo temporal de 15 anos (2005 a 2020), idioma português, tipologia de produção artigos e área temática Ciências da Saúde. Foram acessadas as bases de dados Lilacs, Pepsic, Pubmed e Scielo. Resultado: Foram obtidos 13 artigos, os quais tiveram seus resultados analisados e debatidos. Observou-se que o TB é uma condição psicopatológica grave e com elevado coeficiente de comorbidades. A presença de uma condição comórbide por uso de drogas intensifica o quadro, bem como torna a condução terapêutica de difícil manejo. Aponta-se que 60% a 85% dos pacientes com diagnóstico de TB já fez uso abusivo de alguma droga durante o curso de vida. Dentre as mais utilizadas por pessoas com TB, o álcool representa a substância lícita de maior associação, atingindo cinco vezes mais o paciente com TB frente à população geral. Verificou-se escassez de pesquisas sobre a temática em língua portuguesa.
