Resumo: There’s even a little more than 10 years, physicians advised women with bipolar disorder not to have children. Although this thinking is now outdated, they often still face difficult decisions about how to handle their treatment during pregnancy. Most drugs prescribed for bipolar disorder are associated with some risk of congenital malformations, but patients who discontinue the medication have a high risk of recurrence of a depressive, manic or mixed episode. During the postpartum period, the relapse rateis even higher, reaching 50% to 70% by some estimates. And even more alarming: bipolar women are 100 times more likely than other women to develop postpartum psychosis, a severe condition that can result in maternal suicide and infanticide.

Apesar desse pensamento ser agora ultrapassado, elas ainda enfrentam decisões, muitas vezes difíceis, sobre como lidar com seu tratamento durante a gravidez. A maioria das drogas prescritas para o transtorno bipolar estão associadas a algum risco de malformações congênitas, mas as pacientes que interrompem a medicação têm um alto risco de recaída de um episódio depressivo, maníaco ou misto. Durante o período puerperal, a taxa de recaída é ainda maior, chegando a 50% a 70%, segundo algumas estimativas.
