Nutritional status and pre-operative fasting time for oncological patients submitted to surgery.

Resumo: ABSTRACTObjective: To analyze the nutritional status and the preoperative fasting time of oncological patients undergoing surgical procedures. Method: Quantitative approach study conducted with patients diagnosed with head and neck cancer and/or gastrointestinal tract submitted to surgery during the month of November 2020. A questionnaire was applied to collect clinical, surgical and therapeutic data and a nutrition evaluation was carried out through anthropometry (weight, height, calf circumference) and ASG-PPP tool application. The variables were analyzed according to the nature of the data. Results: Twenty patients with a average age of 62.55 years were evaluated, 55% were female. 80% had moderate or severe malnutrition acording to ASG-PPP. The average preoperative fasting time was 14.21 hours. The average time between diagnosis and surgery was 158 days. There was no statistical difference between the duration of the therapeutic trajectory and the nutritional diagnosis. Conclusion: The preoperative fasting time exceeds the guidelines of 8 to 12 conventional hours and 80% of patients had moderate or severe malnutrition according to the ASG-PPP tool. It is evident the need to develop protocols for shortening the preoperative fasting time, providing better conditions for postoperative recovery. Keywords: Fasting. Neoplasy. Nutritional status. Medical oncology. Objetivo: Analisar o estado nutricional e o tempo de jejum pré-operatório de pacientes oncológicos submetidos a procedimentos cirúrgicos. Método: Estudo de abordagem quantitativa, realizado com pacientes com diagnóstico de câncer de cabeça e pescoço e/ou trato gastrointestinal submetidos à cirurgias durante o mês de novembro de 2020. Foi aplicado questionário para coleta dos dados clínicos, cirúrgicos e de trajetória terapêutica e realizada avaliação nutricional por meio da antropometria (peso, estatura, circunferência da panturrilha) e aplicação da Avaliação Subjetiva Global Produzida pelo Próprio Paciente (ASG-PPP). As variáveis foram analisadas de acordo com a natureza do dados. Resultados: Foram avaliados 20 pacientes com média de idade de 62,55 anos, sendo 55% do sexo feminino e 80% apresentaram desnutrição moderada ou grave pela ASG-PPP. O tempo médio de jejum pré-operatório foi de 14,21 horas. O tempo médio entre o diagnóstico e a  realização da cirurgia foi de 158 dias. Não houve diferença estatística entre o tempo de trajetória terapêutica e o diagnóstico nutricional. Conclusão: O tempo de jejum pré-operatório ultrapassou as orientações de 8 a 12 horas convencionais e 80% dos pacientes apresentavam desnutrição moderada ou grave segundo a ferramenta ASG-PPP. Evidencia-se a necessidade de desenvolvimento protocolos de abreviação do tempo de jejum pré-operatório, proporcionando melhores condições para a recuperação pós-operatória. Palavras-chave: Jejum.
