Acidente vascular cerebral na pediatria: análise da incidência, fatores de risco e manejo em hospital terciário de Curitiba

Resumo: Introduction: Stroke is a comorbidity that can occur at any stage of life, including in the pediatric population, being the most common risk factors for the development of this event vascular diseases and cardiac causes. The clinic is imprecise and the diagnosis is often made lately, resulting in great morbidity and mortality for these patients. Objective: To evaluate the incidence of pediatric stroke in a reference center in Curitiba-PR, as well as evaluating the main risk factors for the event, the management and the evolution of these patients. Method: A retrospective study was made at Associação Hospitalar de Proteção à Infância Dr. Raul Carneiro – Hospital Pequeno Príncipe – through the analysis of medical records of patients who were hospitalized and diagnosed with stroke in the hospital between the period of January 1, 2015 to December 31, 2019. Data collection was made between May to August 2021. These medical records were reviewed, assessing gender, age at which the stroke occurred, medications used during the event, underlying comorbidities, type of stroke, image exam reported, management adopted and the evolution of these patients. Results: A total of 42 patients diagnosed with stroke were found, with the median age at diagnosis of 6 years. Half were male and half were female. There was a preponderance of ischemic stroke, corresponding to 95% of the cases and it was managed with ASA in 47,5% of time. The most common comorbidities found in those patients were heart diseases and cardiac surgery, and 20 patients had any kind of sequel after the event. Conclusion: This is a rare event in the pediatric population, registering 42 cases over a 5-year period in our service. There was a preponderance of ischemic events and the main etiologies were idiopathic, cardiac disease and cardiac surgery prior to the event. Therapeutic management is based on expert opinion and is individual to each service and did not show any change in outcomes in this study. More studies on the subject are needed to correlate risk factors and the management of these patients.

A clínica é imprecisa e muitas vezes o diagnóstico é feito tardiamente, acarretando em grande morbimortalidade para esses pacientes. Objetivo: Avaliar a incidência do acidente vascular cerebral (AVC) na população pediátrica em um centro de referência em Curitiba-PR, bem como avaliar os principais fatores de risco para o evento, o manejo e a evolução desses pacientes. Método: Foi realizado um estudo retrospectivo na Associação Hospitalar de Proteção à Infância Dr. Raul Carneiro – Hospital Pequeno Príncipe – através da análise de prontuários de pacientes que tiveram o internamento e o diagnóstico de acidente vascular encefálico no hospital entre o período de 01 de janeiro de 2015 a 31 de dezembro de 2019. A coleta de dados foi realizada no período de maio a agosto de 2021. Esses prontuários foram revisados avaliando sexo, idade em que ocorreu o AVC, medicamentos em uso durante o evento, comorbidades de base, tipo do AVC, laudo do exame de imagem, conduta adotada e evolução desses pacientes. Resultados: Foi encontrado um total de 42 pacientes com o diagnóstico de AVC, sendo a mediana de idade dos pacientes ao diagnóstico de 6 anos e sendo metade do grupo do sexo masculino e a outra metade do sexo feminino. Houve uma preponderância de AVC isquêmico, correspondendo por 95% dos casos e tendo seu manejo com AAS em 47,5% desses pacientes. As comorbidades e fatores de risco mais encontrados nesses pacientes foram doenças cardíacas e cirurgias cardíacas e 20 pacientes apresentaram alguma sequela decorrente do evento. Conclusão: Esse é um evento raro na população pediátrica, registrando 42 casos em um período de 5 anos em nosso serviço. Houve preponderância nos eventos isquêmicos e as principais etiologias foram idiopática, doença cardíaca e cirurgia cardíaca prévia ao evento. O manejo terapêutico é pautado em opinião de especialistas e é individual de cada serviço e não demonstrou mudança nos desfechos neste estudo.
