Resumo: | In Brazil, the prevalence of contraceptive use by women is over 80%. The birth control pill was introduced in the country in the 1960s and was little known. Subsequently, other contraceptives of high efficacy, low associated effects and which did not depend on daily use, were called long-term reversible contraceptive methods (LARCs), which include copper IUDs, levonorgestrel IUDs and etonogestrel-releasing implants. The objective of this study is to identify and describe the importance of including long-term reversible contraceptive methods in the Unified Health System. This is a bibliographic, qualitative and descriptive review, which included 13 studies published in the last 10 years, in english. or portuguese, which contained the descriptors "contraceptive devices", "contraceptive effects", "unplanned pregnancy" and "reversible long- active contraception", science in the Scielo, PubMed and LILACS databases. Most of the articles included (five) show the main reasons for the low prevalence of the use of long-term reversible contraceptive methods. Four articles present the effectiveness of LARCs in relation to other contraceptives and, finally, four articles study the failure of contraceptive methods of duration with unplanned pregnancies. Note that the high failure rates of action contraceptives are related to the prevalence of unwanted pregnancies. The main reasons for the low use of LARC were the population's lack of knowledge about the contraceptive method and the high cost. It was also found that the free offer of LARCs increased the prevalence of its use and decreased possible unplanned pregnancies. |

A pílula contraceptiva foi inserida no país na década de 60, e era um método pouco conhecido. Posteriormente, surgiram outros contraceptivos com alta eficácia, baixos efeitos colaterais e que não dependem de uso diário, sendo denominados de métodos contraceptivos reversíveis de longa duração (LARCs), os quais englobam o DIU de Cobre, SIU de levonorgestrel e implante liberador de etonogestrel. O objetivo desse estudo é identificar e descrever a importância da inclusão de métodos contraceptivos reversíveis de longa duração no Sistema Único de Saúde. Trata-se de uma revisão bibliográfica, qualitativa e descritiva, em que foram incluídos 13 estudos publicados nos últimos 10 anos, nos idiomas inglês ou português, que continham os descritores “contraceptive devices”, “contraceptive effectiveness”, “unplanned pregnancy” e “long-acting reversible contraception”, aplicados nas bases de dados Scielo, PubMed e LILACS. A maioria dos artigos incluídos (cinco) evidencia as principais razões para a baixa prevalência do uso dos métodos contraceptivos reversíveis de longa duração. Quatro artigos mostram a efetividade dos LARCs em relação a outros contraceptivos, e por fim, quatro artigos estudam a relação da falha dos métodos contraceptivos de curta duração com a gravidez não planejada. Observou-se que as altas taxas de falhas dos contraceptivos de curta ação têm relação com a prevalência de gravidez indesejada. Já as principais razões para o baixo uso do LARC foram o desconhecimento da população acerca desse método contraceptivo e o alto custo.
