Influência da especialidade do profissional no diagnóstico e tratamento de primeiros molares permanentes jovens com lesões de cáries profundas

Dental caries is a biofilm- and sugar-dependent disease that affects the global population. Extensive caries in the dentin can compromise the root canal system because of the progression of the carious process associated with non-attendance of dental appointments. Endodontic diagnosis and treatment can be challenging in children because of the perception of painful stimuli and overestimation of sensitivity. However, little is known about the influence of different specialties in the diagnosis and decision making during the management of young individuals with possible pulp involvement of the permanent first molars. Therefore, the present study aimed to evaluate the influence of clinical specialties in the diagnosis and treatment plan of permanent first molars with deep caries in young individuals. The study sample comprised of 50 endodontists and 48 pediatric dentists registered in CROSP. An online questionnaire prepared on Google Forms was sent with closed-ended questions directed at the diagnosis and treatment of permanent molars with deep caries. They were presented with 14 cases of children of both genders, aged between 9 to 12 years, including their clinical and radiographic images and data on the signs and symptoms of permanent first molars with deep caries. The data were collected from the Pediatric Dentistry Department of Cruzeiro do Sul University. Two examiners reached the consensus on correct answers. The data were analyzed using the chi-squared or Fisher's exact, Mann–Whitney, and Kruskal–Wallis tests, with a 10 subsequent Poisson regression analysis (α=5%). The analysis of the total score for correct answers showed statistically significant differences between the endodontists (19.0 ± 3.5) and pediatric dentists (17.3 ± 3.3) (p=0.014), especially in the diagnosis of pulp changes. There were no differences in the distribution of scores according to the training profile or professional practice of the dentists (p>0.05). Based on the adjusted model, we found that the endodontists had a significantly higher level of accuracy compared to the pediatric dentists (MR=1.139; 95% CI =1.055–1.230; p=0.001). We concluded that the dental specialty influenced the diagnosis and treatment plan of deep caries in the permanent first molars of young individuals, with a greater accuracy on the part of endodontists.
