Limits and potentialities of the accommodation of users with mental suffering in primary care of the Federal District: report of experience in a basic health unit

Resumo: The experience report has as its object of study the organization of user embracement with mental health demands in a Basic Health Unit - UBS in the Federal District, aiming to identify the challenges and potential of this user embracement in primary care. Based on experiences in the Reception room of the UBS, reflections are made about how the team and service are understood as a care device also in mental health. It was observed that the Welcoming carried out by the professionals consisted of a kind of screening, where blood pressure was measured, a protocol of questions was followed and little qualified listening and acceptance of the real demands that arose there were required. It was also found that most users who complained about mental suffering or even cried during the Welcoming were instructed to seek care in the room of the Expanded Family Health Center - NASF, without being assisted by the Health Strategy teams of the Family - ESF. In view of these observations, reflections were also raised on the problems of work processes at the UBS that permeate the difficulties in Welcoming, especially with regard to the demands of mental health. Therefore, it is concluded that Primary Health Care is very powerful in solving the main demands in mental health that arise in the territory through its capacity for intervention and accountability to increase the problem-solving capacity of care with the support of the NASF. However, there is still a way to go in terms of training teams to reduce the fragmentation of care provided and strengthen the capacity for bonding between staff and users.

However, there is still a way to go in terms of training teams to reduce the fragmentation of care provided and strengthen the capacity for bonding between staff and users.O relato de experiência tem como objeto de estudo a organização do Acolhimento aos usuários que apresentam demandas de saúde mental em uma Unidade Básica de Saúde - UBS do Distrito Federal, tendo como objetivo identificar os desafios e potencialidades deste Acolhimento aos usuários na Atenção Primária. A partir de vivências na sala de Acolhimento da UBS, são feitas reflexões acerca de como equipe e serviço se compreendem como um dispositivo de cuidado também em saúde mental. Foi observado que o Acolhimento realizado pelos profissionais consistia em uma espécie de triagem, onde se aferia pressão arterial, se cumpria um protocolo de perguntas e pouco se tinha de escuta qualificada e acolhimento às reais demandas que ali surgiam. Também foi constatado que a maioria dos usuários que apresentavam queixas referentes a sofrimento mental ou que até mesmo choravam durante o Acolhimento, eram orientados a buscarem atendimento na sala do Núcleo Ampliado de Saúde da Família - NASF, sem serem atendidos pelas equipes de Estratégia de Saúde da Família - ESF. Diante dessas observações também foram levantadas reflexões sobre as problemáticas dos processos de trabalho na UBS que perpassam as dificuldades no Acolhimento, principalmente no que se refere às demandas de saúde mental. Conclui-se, portanto, que a Atenção Primária em Saúde é muito potente na resolução das principais demandas em saúde mental que surgem do território por meio da sua capacidade de intervenção e responsabilização para ampliar a resolutividade da assistência com o apoio do NASF.
