Resumo: Introduction: Portal venous system aneurysms represent 3% of all venous aneurysms. The most common sites at which portal venous system aneurysms develop are the main portal vein and the confluence of the splenic and the superior mesenteric veins, followed by hepatic hilus. Objective: Report a case and review the literature about this uncommon aneurysm. Case report: A 76-year-old woman with mental confusion and difficulty walking for one hour which abdominal ultrasound detected an increase in the caliber of the portal vein in the hepatic hilum with important collateral vascularization and intrahepatic portal vein aneurysm splenomegaly with the splenic vein of increased caliber, characterizing portal hypertension. Conclusion: Intrahepatic portal vein aneurysm is an uncommon lesion diagnosed by bedside ultrasonography without the need to transfer the patient for computed tomography.

Os locais mais comuns nos quais os aneurismas do sistema venoso portal se desenvolvem são a veia porta principal e a confluência das veias esplênica e mesentérica superior, seguidas pelo hilo hepático. Objetivo: Relatar um caso e revisar a literatura acerca deste aneurisma incomum. Relato do Caso: Mulher de 76 anos com quadro de confusão mental e dificuldade para deambular há uma hora, cuja ultrassonografia abdominal detectou aneurisma da veia porta intra-hepática, com aumento do calibre no hilo hepático, importante vascularização colateral e esplenomegalia com veia esplênica de calibre aumentado, caracterizando hipertensão portal. Conclusão: O aneurisma intra-hepático da veia porta é uma  lesão incomum, diagnosticada pela ultrassonografia à beira do leito, sem a necessidade de deslocamento da paciente para a tomografia computadorizada.Palavras Chave: Aneurisma, Veia porta, Diagnóstico, Ultrassonografia ABSTRACT Introduction: Portal venous system aneurysms represent 3% of all venous aneurysms.
