Resumo: | The increase in urban hospital discharges in Brazil during the last decade and, consequently, health care costs led the federal government to take several "rational" measures in an attempt to slow the rate of increase in hospital admissions. One of these measures was the introduction of a case-based prospective payment system that seemed to have failed in reducing hospital utilization. The new method of payment seemed to have contributed to increase and not to decrease seen as a good managerial tool for use in comparing hospital performance. However, it seems not to be a good mechanism for controlling health care expenditures, especial1y where private hospitals dominate the provision of hospital care. On the other hand, the introduction of a case-based reimbursement method in Brazil, late in 1983, seemed to have induced private hospitals to admit less severely ill patients, based on the major medical specialties. In short, there have been a change in hospital case-mix, as private hospitals shifted from more costly to less costly patients. |
