Socio-political context and the organization of the workforce and the provision of primary care services

Resumo: In order to analyze the repercussions of the socio- political and normative context for the organization of work in primary care, considering the composition, forms of supply of workers and the perception of managers about performance according to the type of bond, a qualitative study was carried out in 16 municipalities in the northern macro-region of Paraná, Brazil. Coordinators and health secretaries were interviewed from July to October 2021. The findings were presented in three categories: organization of primary care; forms of hiring professionals; and perception of managers about the workers’ performance according to the work bond. There is a preponderance of the Family Health Strategy in the organization of work in the 16 municipalities studied, in which there was the existence of primary care teams in only three. Regarding the forms of hiring, there was an increase in precarious bonds. Regarding the managers perception regarding the performance related to the type of bond, it is evident the idea that outsourced professionals present greater dedication when compared to those who have stability. The socio- political and normative context interfered in the organization and form of provision of professionals, as well as in the managers perception regarding performance and work bond.

Se entrevistó a coordinadores y secretarios de salud de julio a octubre de 2021. Los resultados se presentaron en tres categorías: organización de la atención básica; formas de contratación de los profesionales; y percepción de los gerentes sobre el desempeño de los trabajadores según el vínculo laboral. Hay preponderancia de la Estrategia de Salud de la Familia en la organización del trabajo en los 16 municipios estudiados, en los que se ha comprobado la existencia de equipos de atención primaria en sólo tres. En cuanto a las formas de contratación, hubo un aumento de vínculos precarios. En cuanto a la percepción de los gerentes sobre el desempeño relacionado con el tipo de vínculo, es evidente la idea de que los profesionales de terceros muestran una mayor dedicación en comparación con los que cuentan con estabilidad. El contexto sociopolítico y normativo ha interferido en la organización y la forma de la promoción de los profesionales, así como en la percepción de los administradores en relación con el desempeño y la vinculación del trabajo.Com o objetivo de analisar as repercussões do contexto sociopolítico e normativo para a organização do trabalho na Atenção Básica, ao se considerarem a composição, as formas de provimento dos trabalhadores e a percepção de gestores sobre o desempenho conforme o tipo de vínculo, realizou-se um estudo qualitativo em 16 municípios da macrorregião Norte do Paraná. Foram entrevistados coordenadores e secretários de saúde de julho a outubro de 2021. Os achados foram apresentados em três categorias: organização da Atenção Básica; formas de contratação dos profissionais; e percepção dos gestores sobre o desempenho dos trabalhadores conforme o vínculo de trabalho. Há preponderância da Estratégia Saúde da Família na organização do trabalho nos 16 municípios estudados, nos quais se verificou a existência de equipes de Atenção Primária em apenas três. Sobre as formas de contratação, houve aumento de vínculos precários. Quanto à percepção dos gestores a respeito do desempenho relacionado ao tipo de vínculo, fica evidente o ideário de que profissionais terceirizados apresentam maior dedicação quando comparados àqueles que contam com estabilidade. O contexto sociopolítico e normativo interferiu na organização e na forma de provimento dos profissionais, assim como na percepção dos gestores em relação ao desempenho e vínculo de trabalho.
