From the specific actions to government mark: medicine in the trajectory of the national policy of education and work regulation in health

Resumo: This article analyzes the trajectory of the Brazilian policy of education and work regulation considering institutional issues and forms of incorporation of medicine on the policy, comprising the period from 2003 to 2015. This study was based on contributions of historical institutionalism, involving the analysis of legal and normative documents, interviews with state personnel and data from federal financing reports of the policy. Five key moments were defined, considering political-institutional contexts, governmental initiatives, and the insertion of medicine into the agenda of governments. The emphasis on the medical profession evolved from one-off actions to the status of a landmark, in a context of loss of space of the health workforce agenda and increasing prioritization of education actions, associated with institutional gains and changes in funding. The More Doctors Program resulted from the dynamics of the trajectory, incorporating advances in the training and incorporation of physicians into the Unified Health System of Brazil. Challenges remain, however, related to precarious employment relationships, the privatization of medical education, and professional regulation that considers the exercise of medicine in the private sector.

El estudio se basó en contribuciones del institucionalismo histórico, que implicaban el análisis de documentos legales y normativos, entrevistas con actores statales y datos de financiación de políticas federales. Se identificaron cinco momentos, basados en el contexto político-institucional, así como iniciativas gubernamentales y la inserción de la medicina en las agendas gubernamentales. El énfasis en la profesión médica evolucionó de acciones específicas, a la condición del marco gubernamental, dentro del contexto de la pérdida de espacio de la agenda de trabajo en salud y el aumento de la priorización de acciones educativas, asociadas con avances en las instituciones y cambios en la financiación. El Programa Más Médicos fue el resultado de la dinámica de la trayectoria recorrida, con avances en la formación y provisión de médicos en el SUS. Siguen existiendo desafíos respecto a las relaciones laborales precarias, privatización de la educación médica, así como la regulación profesional, en lo que concierne al ejercicio de la medicina en el sector privado.O artigo tem como objetivo analisar a trajetória da política nacional de educação e regulação do trabalho na saúde, considerando a institucionalidade e formas de incorporação da medicina no percurso da política, de 2003 a 2015. O estudo se baseou em contribuições do institucionalismo histórico, envolvendo análise de documentos legais e normativos, entrevistas com atores estatais e dados do financiamento federal da política. Cinco momentos foram identificados com base no contexto político-institucional, nas iniciativas governamentais e na inserção da medicina nas agendas de governo. A ênfase na profissão médica evoluiu de ações pontuais à condição de marco governamental, no contexto de perda de espaço da agenda do trabalho em saúde e priorização crescente das ações de educação, associada a ganhos de institucionalidade e mudanças no financiamento. O Programa Mais Médicos resultou da dinâmica da trajetória percorrida, com avanços na formação e no provimento de médicos no Sistema Único de Saúde.
