Progressive supranuclear palsy (Steele-Richardson-Olszewski syndrome) associated to generalized seizures: case report and revision of literature.

Resumo: Introduction: Progressive Supranuclear Palsy is a neurodegenerative disease that affects mainly the brain stem and basal ganglia. The clinical picture is characterized by supranuclear ophthalmoplegia, postural instability and dementia. Methods: Analysis of medical records and literature review. Case Report: Man, 74 year-old, developed parkinsonism about 16 years earlier, with progressive evolution; initially he had postural abnormality, instability of gait and bradykinesia, that evolved to neck stiffness, projected backwards, and evident dementia. With diagnosis of Parkinson's Disease, he was treated with Levodopa, without clinical improvement over the years. Nearly eight months ago he had an episode of generalized tonic-clonic seizure, subsequent recurrence until institution of anticonvulsant therapy. Conclusion: Progressive Supranuclear Palsy is a neurodegenerative disease, with inexorable evolution, unresponsive to medical treatment. It affects mainly men, in the fifth decade of life, being often confused with Parkinson's Disease. Thepresentation is classic, with predominant ocular supranuclear palsy and poorly responsive to Levodopa therapy and reports of seizures or disorders of consciousness are rarely reported in the series of cases studied.

O quadro clínico se caracteriza por oftalmoparesia supranuclear, instabilidade postural e demência. Método: Análise de prontuário e revisão da literatura. Descrição do caso: Homem, 74 anos, iniciou quadroparkinsoniano há cerca de 16 anos, com evolução progressiva; inicialmente apresentou alteração postural, instabilidade da marcha e bradicinesia, que evoluiu com enrijecimento do pescoço, com projeção para trás, além de síndrome demencial evidente. Diagnosticado como Doença de Parkinson, foi instituído tratamento com Levodopa, sem melhora clínica ao longo dos anos. Há cerca de oito meses apresentou episódio de crise convulsiva tônico-clônica generalizada com recorrência subsequente, até instituição de terapia anticonvulsivante. Conclusão: A Paralisia Supranuclear Progressiva é uma doença neurodegenerativa, de evolução inexorável, pouco responsiva aos tratamentos clínicos. Atinge, sobretudo, homens, a partir da quinta década de vida, sendo muitas vezes confundida com a Doença de Parkinson.
