DIAPRAGMATIC EPIDERMOID CYST – CASE REPORT

Resumo: | Introduction: Non-cutaneous epidermoid cysts are uncommon abnormalities that can be found anywhere in the body, including the diaphragm, an extremely rare place for the occurrence of primary tumors. In general, it may be asymptomatic, however chest or abdominal pain may be referred to. Objective: The aim of the present paper is to describe a case of large diaphragmatic epidermoid cyst. Method: A descriptive study consisting of a case report based on the analysis of medical records, surgical and radiological images, histopathological and immunohistochemical reports, and a review of the literature in the electronic databases. Original scientific articles were obtained from LILAC, SciELO Brazil, Scopus and Pubmed. Case report: Female patient, 46 years old, single, started two days ago with pleurisy pain in the right costal margin and pain in the right hypochondrium, associated with anorexia. Computed tomography of the abdomen was performed, which showed solid-cystic tumor, measuring 20 x 14 x 0.5 cm, right subphrenic, determining mediocaudal hepatic and retraction of the inferior vena cava medially. Therefore, resection was indicated. For better definition of the histogenesis, an immunohistochemical study was performed which elucidated the diagnosis of epidermoid cyst. Conclusion: In the literature, the classic lesion of the epidermoid cyst is described as being of cutaneous origin, making the non-cutaneous type uncommon. Among these, the cases described in the literature of diaphragmatic epidermoid cyst are rare. The reported case of this type of large cyst shows that location and size may determine the onset of symptoms and even organ retractions. |

Em geral, apresentam-se assintomáticos, porém dor torácica ou abdominal pode ser referida. Objetivo: Descrever um caso de cisto epidermóide diafragmático de grandes dimensões. Relato do caso: Paciente sexo feminino, 46 anos, solteira, há dois dias iniciou um quadro de dor pleurítica em rebordo costal direito e dor em hipocôndrio direito associada à anorexia. Realizada tomografia computadorizada do abdomen a qual evidenciou tumoração sólido-cística, medindo 20 x 14 x 0,5 cm, subfrênica direita, determinando retração hepática e mediocaudal da veia cava inferior medialmente. Sendo assim, foi indicada ressecção. Para melhor definição da histogênese, foi realizado estudo imunohistoquímico o qual elucidou o diagnóstico de cisto epidermóide. Conclusão: Na literatura, é descrita a lesão clássica do cisto epidermóide como sendo de origem cutânea, tornando incomum o tipo não cutâneo. Dentre esses, são raros os casos descritos na literatura de cisto epidermóide diafragmático.
