Iliopsoas abscess in child with probably hematogenic etiology: case report

Resumo: | Introduction: The abscess of psoas is a rare entity in the medical practice, with either primary or secondary etiology.The iliopsoas muscle has a direct relation to the ileocecal junction, the vertebral column and the renal tract, so intestinal diseases, osteomyelitis of the spine and urinary tract infections are the most implied disorders of an abscess of psoas. Clinical diagnosis is difficult due to nonspecific clinical manifestations. The computed tomography is the gold standard method of diagnosis with typical findings since the beginning of symptoms. Objective: To describe a case of iliopsoas abscess in a child with probably hematogenic etiology. Method: Review of medical records. Case report: A 6-year-old girl was admitted with back pain and difficulty for walking since twenty days prior consultation, which was not responsive to the use of anti-inflammatory therapy. She developed fever and received Cephalexin treatment. Without improvement, she was admitted for investigation and she had the diagnosis of iliopsoas abscess confirmed by computed tomography. Conclusions: Although rare the infection of psoas must be remembered in the presence of pain around groin, guarding against hip motion, fever, elevated ESR and CRP and after the evaluation for arthritis was negative. |

O músculo ílio-psoas tem relação direta com a junção íleocecal, com a coluna vertebral e com o trato urinário, de forma que patologias intestinais, osteomielite de coluna e infecções do trato urinário são as desordens mais comumente implicadas na etiologia da psoíte. A possibilidade de origem hematogênica também deve ser considerada. O diagnóstico clínico é difícil, pois as manifestações são inespecíficas. A tomografia computadorizada é o método de eleição para a confirmação do diagnóstico, com alterações típicas mesmo no início dos sintomas. Objetivo: Relatar o caso de um abscesso de ílio psoas de provável origem hematogênica, em escolar. Método: Revisão de prontuário. Relato de caso: Menor de 6 anos de idade foi admitida com dor na região lombar e dificuldade para deambular há 20 dias, que não cedeu com o uso de antiinflamatório. Evoluiu com o aparecimento de febre, sendo instituída a Cefalexina. Como não houvesse regressão das queixas foi aconselhada a internação, durante a qual se confirmou a presença de abscesso de ílio psoas pela tomografia computadorizada.
