ACUTE AND CHRONIC EFFECT OF ESTRADIOL ON BLOOD PRESSURE IN POSTMENOPAUSAL WOMEN WITH CLASS II HYPERTENSION

Resumo: | Background: hormonal replacement therapy (HRT) is widely used to relie ve menopausa[ symptoms and to prevent and treat osteopo rosis. But its effect on blood pressure in hypert ensive postmenopausal women is unknown. Objectives: To evaluate the acute and chronic effects of estradiol, in physiol ogic doses, in postmenopausal women withclass li hypert en sion. Methods: Thirty four hypertensive postmenopausal women, mean age 58, 58 ± 3,5 years old , prospecti vely double blind randomized to receive placebo (n=17) or oral estradiol (n= 17), at dose of 1 mg per day, during 1.2 consecutive weeks. Blood pressur e was measured during basel in e, 90 minut es and 12 weeks after estradiol or placebo administ rat ion, usi ng automatic device the antihipertensive medication didn't change durin g the st udy. Results: baseli ne, 90 minute s and 12-weeks e st radiol levels 20 ± O, 85 ± 50 and 120 ± 74 pg/ ml (p <0,001) respectively in est r adiol group . ln pl acebogroup the estr adiol level was 20 ± O p g/ ml in the remoments. Baseline, 90 minutes and 12-weeksblood pressur e was 153 ± 22, 152 ± 17 and 149 ± 16 mmHgin estradiol groupand 95 ± 12, 91 ± 10 and 93 ± 9 mmHgin placebo group, respectively (p <ns). Conclusions:no diff erences statisti cs significant intra-group and among the groupsest radiol and placebo on blood pressur e in the three moments of t he st udy, what all owed to conclude us t hat HRT may not be harmful in hypertensive post menopausal women usinganti hypertensives medic ations. |

Objetivo: Avaliar os efeitos agudos e crônicos do estradiol, em doses fi siol ógicas, sobre a pressão arterial sistêmica em mulheres após a menopausa com hipertensão arterial estágio li e em tratamento. Metodologia: Foram avaliadas 34 mulheres com média etária de 58,5 8 ± 3, 5 anos, dist ribuída s em dois grupos: 17 usuárias de estradiol(1 mg, via oral)denominadas grupo est radiol (GE) e 17 usuárias de placebo, chamadas de grupo placebo (GP). O estudo teve duração de12 semanas e foi do t ipo prospectivo, randomizado, duplo-cego. A pressão arterial foi medida usando um aparelho automático e de forma contínua, no momento basal, após 90 minutos e 12 semanas da administração do estradiol e do placebo. Todas estavam em uso de medicação anti-hipertensiva como os inibidores da enzima conversora do angiotensinogênio (ECA), diuréticose alfa-metildopa. Resultados: As concentrações séricas do estradiol nos três momentos do estudo foram de 20 ± O, 85 ± 50, 120 ± 74 pg/ml (p<0,001) no GE e no GP foi igual 20 ± O pg/ml no momento basal, 90 minutos e 12 seman as após tratamento. A pressão arterial sistólica (PAS) no grupo estradiol foi de 153 ± 22, 152 ± 17 e 149 ± 16 mmHg no momento basal, após 90 minutos e 12 semanas após tratamento , respectivamente (p < ns ) e a pressão arterial diastólica (PAD) foi de 95 ± 12, 91 ± 10 e 93 ± 9 mmHg no momento basal, após 90 minutos e 12 semanas após o tratamento, respectivamente( p<ns). Conclusões: Não ocorr eram diferenças estatisticamente significantes int ra-grupo e entre os grupos estradiol e placebo para PAS e PAD nos três momentos do estudo, o que nos permitiu concluir que mulheres após a menopausa, hipertensas e sob t erapia anti-hipertensiva não exibem contra- indicação à THM com estradiol administrada durante 12 semanas .
