Ophthalmological adverse effect through the use of hydroxychloroquine and its analogues: a bibliographical review

Resumo: Chloroquine (CQ) and hydroxychloroquine (HCQ), due to their beneficial effects, have been widely used in several medical therapy scenarios. However, the important side effects associated with these drugs is the potential ophthalmic injury, specifically to the retina. Primarily, CQ was developed for treatment of falciparum malaria, however, in the current scenario it is widely used in several medical areas. The risk of toxicity of this drug depends on the dose and duration of therapy, and side effects reactions can reach the gastrointestinal tract, hematological, neurological, neuromuscular, dermatological and cardiological system, however, the ocular facts the most worrying, being the most known clinical characteristic of retinopathy by HCQ the bilateral maculopathy. To minimize its side effects, it is necessary to perform a screening of patients before starting HCQ or CQ therapy, where an initial examination should be performed to assess any complicated ocular conditions, the degree of risk and establish a record of the appearance of the fundus and visual field. Until the moment, no drug therapy tested has been effective in preventing or treating HCQ or CQ-induced retinopathy. It is important monitorization the patients using these medications. Despite the relatively low risk these damages may be irreversible, but when detected early, the greater chances are that the visual capacity will be preserved when stopping treatment.

Despite the relatively low risk these damages may be irreversible, but when detected early, the greater chances are that the visual capacity will be preserved when stopping treatment.A cloroquina (CQ) e a hidroxicloroquina (HCQ), devido a seus efeitos benéficos, têm sido amplamente utilizados em diversos cenários de terapêutica médica. No entanto, deve-se levar em consideração os importantes efeitos adversos associados à essas drogas, sendo o mais temido o potencial dano oftalmológico, especificamente ao nível da retina. Inicialmente, a CQ foi desenvolvida para o tratamento da malária falciparum, no entanto, no cenário atual é amplamente utilizada em diversas áreas médicas. O risco de toxicidade dessa droga depende da dose diária e da duração do uso, sendo que suas reações adversas podem atingir o trato gastrointestinal, sistema hematológico, neurológico, neuromuscular, dermatológico e cardiológico, sendo, no entanto, os feitos oculares os mais preocupantes, sendo a característica clínica mais conhecida da retinopatia por HCQ a maculopatia bilateral em olho de boi. A fim de minimizar seus efeitos adversos, é preciso que seja feita uma triagem dos pacientes antes do início da terapia com HCQ ou CQ, onde deve ser realizado um exame inicial para avaliar quaisquer condições oculares complicadas, o grau de risco e estabelecer um registro da aparência do fundo e campo visual. Até o momento, nenhuma terapia medicamentosa testada apresentou-se eficaz na prevenção ou no tratamento da retinopatia induzida por HCQ ou CQ. É imprescindível a importância de um acompanhamento dos pacientes em uso dessas medicações.
