Effects of the description of proton pump inhibitors

Resumo: | Introduction: Among the most prescribed drugs worldwide are proton pump inhibitors (PPIs), which arevery useful in the treatment of gastric diseases and are effective because of their low toxicity and blockage of gastricsecretion. Despite its usefulness during treatment, the prolonged use of these drugs can cause serious complications andlosses for the patient who makes continuous use of these drugs. In order to reduce the dose or interrupt the use of drugs thatcan cause harm, much has been invested in the description of PPI, which must be carried out in order to consider thebenefits, the purpose of the treatment, the convenience, the age and also patient cooperation. Objective: To carry out aliterature review regarding the negative and positive effects of the description of proton pump inhibitors. Methods:Narrative review regarding the description of the IBP carried out at the Virtual Health: Library (VHL) and in the SCIELO,LILACS databases, using the descriptors: description, toxicity, gastric acid and esophagitis. 15 articles published in thelast 12 years were selected, involving the three thematic nuclei: Action of proton pump inhibitors under gastric secretion,complications due to the prolonged use of PPIs and the importance of the description of PPIs. Results: It is known thatseveral patients end up self-medicating or continuing previous treatments without medical knowledge; in addition, the useof many drugs together without prior analysis for the pathology can cause several complications. Thus, the prescription ofthese drugs, especially PPIs, is one of the ways to prevent unwanted effects, diseases in the patient and even complicationsof pre-existing diseases. The importance of prescribing is to prevent PPIs from being prescribed indefinitely and also toensure patient safety by avoiding exposure to the inherent risks of adverse reactions, medication errors, drug interactionsand hospitalizations due to complications for the same. Conclusion: In this sense, although PPIs are potent for gastricdiseases, they are unnecessary for some digestive diseases, hence the importance of a multidisciplinary team to treat thepatient and reduce the prescription of these PPIs. We conclude that, in order to perform the description, it is necessary,then, to choose the treatment that has a general approach and that brings less harm to the patient's health and life.Keywords: Description, Toxicity, Gastric acid. Esophagitis.Introdução: Entre os medicamentos mais prescritos mundialmente encontram-se os inibidores da bomba deprótons (IBP), que apresentam bastante utilidade no tratamento de doenças gástricas e eficácia pela sua baixa toxicidade ebloqueio da secreção gástrica. Apesar da sua utilidade durante o tratamento, o uso prolongado destes medicamentos podecausar sérias complicações e prejuízos para o paciente que faz uso contínuo desses fármacos. Com o intuito de diminuir adose ou interromper o uso de medicamentos que possam trazer danos, tem-se investido muito na desprescrição de IBP, quedeve ser realizada de modo a considerar os benefícios, a finalidade do tratamento, a comodidade, a idade e também acooperação do paciente. Realizar uma revisão de literatura a respeito dos efeitos negativos Objetivo: e positivos dadesprescrição dos inibidores de bomba de prótons. Métodos: Revisão narrativa a respeito da desprescrição do IBPrealizada na Biblioteca Virtual de Saúde (BVS) e nos bancos de dados SCIELO, LILACS, usando os descritores:desprescrição, toxicidade, ácido gástrico e esofagite. Foram selecionados 15 artigos publicados nos últimos 12 anos queenvolviam os três núcleos temáticos: Ação dos inibidores de bomba de protóns sob a secreção gástrica, complicações pelouso prolongado de IBP e a importância da desprescrição dos IBPs. Resultados: Sabe-se que vários pacientes acabamautomedicando-se ou dando continuidade a tratamentos anteriores sem o conhecimento médico; além disso, o uso demuitos fármacos juntos sem uma análise prévia para a patologia pode ocasionar várias complicações. Desse modo, adesprescrição desses medicamentos, especialmente dos IBPs, é uma das maneiras de impedir efeitos indesejados,doençasno pacientee até mesmo complicações de doenças pré-existentes. A importância da desprescrição é evitar que IBPs sejamprescritos por tempo indeterminado e também garantir a segurança do paciente evitando sua exposição a riscos inerentesde reações adversas, erros de medicação, interações medicamentosas e internações em decorrência de complicações para omesmo. Conclusão: Nesse sentido, embora sejam potentes para as doenças gástricas, os IBPs são desnecessários paraalgumas doenças digestivas, daí a importância de uma equipe multidisciplinar para tratar o paciente e reduzir a prescriçãodestes IBPs.
