Episiotomy in puerperium: women’s perception

Resumo: | The study aimed to analyze the emotional and sexual aspects that permeate the postpartum period of episiotomized women. This is an exploratory, descriptive research of qualitative nature. Data collection occurred through interviews with ten mothers in the remote postpartum period in March and April 2013, at their homes. The analysis of the interviews led to results that showed the little knowledge of mothers about episiotomy, although not being the first delivery. All of them said they did not care about episiotomy at first, considering it as essential for the birth. The procedure is not questioned, because the women trust their physicians, and the decision-making about episiotomy lies on the physician; women report to the doctor as the main interlocutor in the puerperal pregnancy cycle. The nursing staff did not appear as a reference in the women’s speech, it was referenced only when related to the performance of technical procedures. It is concluded that Nursing, especially the nurse, may be a key professional in the development of new paradigms. Episiotomy is considered a violation of women’s sexual and reproductive rights, thus it is essential to inform and discuss about its veiled consequences with pregnant women. A more committed action by the health professionals, especially nurses, based on scientific knowledge, is imperative for the demystification and rational use of this procedure. |

Trata-se de uma pesquisa exploratória, descritiva de natureza qualitativa. A coleta de dados deu-se por meio de entrevistas com dez puérperas, no período do puerpério remoto, nos meses de março e abril de 2013, em seus respectivos domicílios. A análise das entrevistas levou a resultados que evidenciaram o pouco conhecimento das puérperas sobre a episiotomia, mesmo não sendo o primeiro parto. Todas disseram não se importar com a episiotomia no primeiro momento, considerando-a como algo essencial para o nascimento. O procedimento não é questionado, pois as mulheres depositam confiança no médico, e a tomada de decisão sobre fazer a episiotomia é médica; as mulheres se reportam ao médico como interlocutor principal no ciclo gravídico puerperal. A equipe de Enfermagem não apareceu como referência na fala das mulheres, foi lembrada apenas na realização de procedimentos técnicos. Conclui-se que a Enfermagem, sobretudo a enfermeira, pode ser uma profissional fundamental no desenvolvimento de novos paradigmas. Considera-se a episiotomia uma violação dos direitos sexuais e reprodutivos das mulheres, torna-se imprescindível informar e discutir com as gestantes sobre suas veladas consequências.
