SOCIODEMOGRAPHIC AND CLINICAL CHARACTERIZATION OF MATERNAL DEATHS DUE TO COVID-19 IN A REFERENCE MATERNITY, 2020-2021

Resumo: This study aims toto describe the sociodemographic and clinical characteristics of maternal mortality caused by COVID-19 in a reference maternity hospital in the Northeast of Brazil. It was performed a retrospective, descriptive, case series study making use of the Mortality Data System, hospital investigation forms and death record analysis occurring in the years 2020 and 2021 and accounted by the hospital’s committee on mortality. As a result, There were 24 maternal deaths caused by COVID-19, 22 during pregnancy; in woman median age 30 years (interquartile range: 25,5 – 37,0 years). Most deaths occurred in residents of other municipalities of Piauí (17), with partner (13), had other children (18), had antenatal check-ups (16), with less than six consultations (9), delivery/abortion assisted at a reference maternity hospital (23), by obstetricians (23), deliveries were C-sections (12), assisted in the maternal ICU (24), and had not been vaccinated against COVID-19 (24). In terms of pathologies, 11 of them were obese and 11 had secondary infection to COVID-19. Therefore, There was an increase in maternal deaths caused by COVID-19 between 2020 and 2021, especially in young women, during pregnancy and with comorbidities and clinical complications. Método: estudio retrospectivo, serie de casos, descriptivo utilizando el Sistema de Información de Mortalidad, fichas de investigación y ficha de análisis de las defunciones, analizadas por el comité de defunciones de la maternidad y ocurridas entre 2020 y 2021. Fueron identificadas 24 muertes maternas por COVID-19, 22 durante el embarazo; em mujeres com uma mediana de edad de 30 años (rango intercuartílico: 25,5 – 37,0 años). La mayoría delas muertes ocurrieron en otros municipios de Piauí (17), com pareja (13), multíparas (18), tuvieron consultas prenatales (16), com menos de seis consultas (9), parto/aborto em la sala de maternidad de referencia (23), realizada por obstetras (23), partos por cesárea (12), atendidas en la Unidad de Cuidados Intensivos (24), y no vacunadas contra el covid-19 (24). En cuanto a patologías, 11 eran obesas y 11 tenían una infección secundaria a covid-19. Llegamos a la conclusión de que hubo un aumento de las muertes maternas por covid-19 entre 2020 y 2021, especialmente em mujeres jóvenes, durante el embarazo y con comorbilidades y las complicaciones clínicas. Epidemiología descriptiva.Este estudo tem por objetivo apresentar as características sociodemográficas e clínicas dos óbitos maternos causados por covid-19, em uma maternidade de referência no Nordeste, Brasil. Foi realizado um estudo retrospectivo, do tipo série de casos, descritivo, utilizando dados do Sistema de Informações sobre Mortalidade, fichas de investigação e ficha de análise dos óbitos ocorridos entre 2020 e 2021. As variáveis foram analisadas com a estatística descritiva. Identificaram-se 24 óbitos por covid-19,22 na gravidez; em mulheres com idade mediana de 30 anos (intervalo interquartil: 25,5 – 37,0). A maioria dos óbitos ocorreu em residentes em outros municípios do Piauí (n=17), com companheiro (n=13), multíparas (n=18), fizeram consulta pré-natal (n=16), com menos de seis consultas (n=9), parto/aborto na maternidade de referência (n=23), realizado por obstetras (n=23), via de parto cesárea (n=12); internadas na unidade de terapia intensiva (n=24), não tomaram vacina contra covid-19 (24). Quanto as comorbidades e complicações, 11 eram obesas e 11 apresentaram infecções secundária a covid-19. Houve aumento de mortes maternas entre 2020 e 2021 devido a ocorrência da covid-19, principalmente, em mulheres jovens, durante a gestação e com comorbidades e complicações clínicas.
