Abstract   Introduction: Prolonged retention of deciduous teeth can be an important determinant factor for dental malocclusion. Objective: To report a case of prolonged retention of deciduous teeth. Case Report: Patient ALMP, 11 years old, female, attended the University Center of Pará (CESUPA). Upon clinical and imaging examination, prolonged retention of element 55 was confirmed, which caused mesialization of elements 54 and 13, also causing changes in occlusion with crossbite of the latter. In directed anamnesis, the patient did not report any local or systemic disorders that could justify the anomaly. The patient underwent surgery to remove the ankylosed element, said to be 55, and immediately received orthodontic treatment through the use of a removable appliance, with the aim of uncrossing the bite and favoring the intercuspation of the mesialized elements. Final considerations: It can be concluded that early intervention in prolonged retention, with surgical planning, space maintenance and interceptive orthodontics, favored the installation of a normal occlusion beyond concerns of a general problem.
