ACIDENTE VASCULAR ENCEFÁLICO E USO DA ESCALA NIHSS

Resumo: | Stroke (CVA) is a global problem. It can be defined as a neurological deficit caused, abruptly, by the interruption of blood flow to the brain, in an ischemic or hemorrhagic manner. Ischemic stroke represents around 80% of these events, being the most prevalent and incident in Brazil. It is estimated that around 15 million people are affected each year and of these 5 million die. The aggressiveness of this pathology makes it one of the main causes of death in the world, generating high costs, around 0.66% of global GDP. The NIHSS scale, developed in the 1980s, underwent changes in 2004 to improve its usefulness in light of the Brazilian reality and is accepted as a potential modifier of the care offered to post-stroke patients. The objective of this study was to carry out a brief review of the literature on the applicability of the NIHSS scale in post-stroke patients. To prepare this study, databases such as SciELO, MEDLINE/PubMED and LILACS were used. Data analysis showed that, even with the existence of programs to combat the factors that predispose to this pathology, the incidence of this disease may increase in the coming years and the NIHSS scale, although programmed to assist in research, is accepted as an examination more effective clinical approach to quantify the neurological deficit caused by this condition. The scale also proved to be efficient in determining the best treatment by quickly allowing 5 to 8 minutes to understand the patient's current state: regarding the progression of the neurological deficit. An important factor for applying the scale is the training of professionals, which can be carried out online, however, caution must be taken when applying it, as this tool was designed for research purposes. |

Se puede definir como un déficit neurológico provocado, de forma brusca, por la interrupción del flujo sanguíneo al cerebro, de forma isquémica o hemorrágica. El ictus isquémico representa alrededor del 80% de estos eventos, siendo el más prevalente e incidente en Brasil. Se estima que alrededor de 15 millones de personas se ven afectadas cada año y de ellas 5 millones mueren. La agresividad de esta patología la convierte en una de las principales causas de muerte en el mundo, generando elevados costos, alrededor del 0,66% del PIB global. La escala NIHSS, desarrollada en la década de 1980, sufrió modificaciones en 2004 para mejorar su utilidad ante la realidad brasileña y es aceptada como un potencial modificador de la atención ofrecida a los pacientes post-ictus. El objetivo de este estudio fue realizar una breve revisión de la literatura sobre la aplicabilidad de la escala NIHSS en pacientes post-ictus. Para la elaboración de este estudio se utilizaron bases de datos como SciELO, MEDLINE/PubMED y LILACS. El análisis de los datos mostró que, incluso con la existencia de programas para combatir los factores que predisponen a esta patología, la incidencia de esta enfermedad puede aumentar en los próximos años y la escala NIHSS, aunque programada para ayudar en la investigación, es aceptada como un examen más enfoque clínico eficaz para cuantificar el déficit neurológico causado por esta condición. La escala también demostró ser eficaz para determinar el mejor tratamiento al permitir rápidamente de 5 a 8 minutos comprender el estado actual del paciente: con respecto a la progresión del déficit neurológico. Un factor importante para la aplicación de la escala es la formación de profesionales, la cual se puede realizar en línea, sin embargo, se debe tener precaución al aplicarla, ya que esta herramienta fue diseñada con fines de investigación. O Acidente Vascular Encefálico (AVE) é um problema mundial. Pode ser definido como um déficit neurológico causado, de forma abrupta, pela interrupção do fluxo sanguíneo para o cérebro, de modo isquêmico ou hemorrágico. O AVE isquêmico representa cerca de 80% desses acontecimentos, sendo o mais prevalente e incidente no Brasil. Estima-se que cerca de 15 milhões de pessoas são acometidas por ano e dessas 5 milhões evoluem para óbito. A agressividade dessa patologia faz com que seja uma das principais causas de morte no mundo gerando custos elevados, cerca de 0,66% do PIB global. A escala NIHSS, desenvolvida na década de 80, sofreu alterações no ano de 2004 para melhor utilidade diante da realidade brasileira e é aceita como potencial modificadora do cuidado ofertado para pacientes pós-AVE. O objetivo deste estudo foi realizar uma revisão breve da literatura sobre a aplicabilidade da escala NIHSS em pacientes pós-AVE. Para elaboração deste estudo utilizou-se de bases de dados como SciELO, MEDLINE/PubMED e LILACS. A análise dos dados evidenciou que, mesmo com a existência de programas de combate aos fatores que predispõe a essa patologia, a incidência dessa enfermidade poderá aumentar nos próximos anos e a escala NIHSS, embora tenha sido programada para auxiliar em pesquisas, é aceita como exame clínico mais eficaz para quantificar o déficit neurológico causado por esse agravo. A escala também se mostrou eficiente para determinar o melhor tratamento por permitir, de forma rápida, 5 a 8 minutos a compreensão do estado atual do paciente: quanto a progressão do déficit neurológico. Um fator importante para a aplicação da escala, é a capacitação dos profissionais que pode ser realizada on-line, todavia, deve-se ter cautela ao aplicá-la, pois esta ferramenta foi elaborada para fins de pesquisa.
