Ketamine in acute pain management: an integrative review

Resumo: Ketamine's mechanism of action is to antagonize NMDA receptors (N-methyl-D-aspartate) with analgesic and sedative potential. Introduced in clinical practice about 30 years ago, acting as a monoanesthetic drug. In recent years indications for the use of ketamine have emerged, with discoveries of clinical placements including pain management and use in psychiatry. The main motivation for studying the use of ketamine stemmed from the possibility of making the drug an option in the treatment of pain, since the most used drugs, opioids, can cause complications and dependence. The objective is to analyze how the national and international scientific literature describes the use of ketamine in the management of acute pain. This is an integrative literature review with a qualitative approach, carried out according to the six steps recommended by Ganong. The bibliographic survey was carried out through an online search in the databases available in the Virtual Health Library (VHL), and in the Scientific Electronic Library Online (SciELO), PubMed and Embase databases. Based on Health Sciences Descriptors (DeCS/MeSH), the descriptors in Portuguese ketamina and pain acute, and in English ketamine and acute pain, were used in the selected databases. For the results, the articles selected for this research were analyzed in full format, clinical trial type and grouped into categories "use of ketamine in the emergency", "use of ketamine in postoperative pain" and "use of ketamine in the various pathologies that present acute pain". For critical analysis and discussion, 15 articles composed the study results. It is concluded that Ketamine is an effective adjuvant, as it prolongs the duration of other drugs in the management of acute pain in emergencies, has potential for analgesia, reduction of doses and side effects of opioids, its perioperative use should be considered and its use in low doses presents promising perspectives in postoperative analgesia.

En los últimos años han surgido indicaciones para el uso de ketamina, con descubrimientos de aplicaciones clínicas que incluyen el tratamiento del dolor y su uso en psiquiatría. La principal motivación para estudiar el uso de la ketamina surgió de la posibilidad de hacer del fármaco una opción en el tratamiento del dolor, ya que los fármacos más utilizados, los opioides, pueden provocar complicaciones y dependencia. El objetivo es analizar cómo la literatura científica nacional e internacional describe el uso de ketamina en el manejo del dolor agudo. Se trata de una revisión integradora de la literatura con un enfoque cualitativo, realizada de acuerdo con los seis pasos recomendados por Ganong. El levantamiento bibliográfico se realizó a través de una búsqueda en línea en las bases de datos disponibles en la Biblioteca Virtual en Salud (BVS), y en las bases de datos de la Biblioteca Científica Electrónica en Línea (SciELO), PubMed y Embase. Con base en los Descriptores de Ciencias de la Salud (DeCS/MeSH), en las bases de datos seleccionadas se utilizaron los descriptores en portugués ketamina y dolor agudo, y en inglés ketamina y dolor agudo. Para los resultados, los artículos seleccionados para esta investigación fueron analizados en formato completo, tipo ensayo clínico y agrupados en categorías "uso de ketamina en la emergencia", "uso de ketamina en el dolor postoperatorio" y "uso de ketamina en las diversas patologías que presentan dolor agudo". Para el análisis crítico y la discusión, 15 artículos comprendieron los resultados del estudio. Se concluye que la Ketamina es un coadyuvante eficaz, ya que prolonga la duración de otros fármacos en el manejo del dolor agudo en emergencias, tiene potencial para analgesia, reducción de dosis y efectos secundarios de los opioides, se debe considerar su uso perioperatorio y su uso. A principal motivação para estudar o uso da ketamina decorreu da possibilidade de tornar a droga uma opção no tratamento da dor, visto que as drogas mais utilizadas, os opióides, podem causar complicações e dependência. O objetivo é analisar como a literatura científica nacional e internacional descreve a utilização da ketamina no manejo da dor aguda. Trata-se de uma revisão integrativa da literatura com abordagem qualitativa, realizada de acordo com as seis etapas recomendadas por Ganong. O levantamento bibliográfico foi realizado em busca online nas bases disponíveis na Biblioteca Virtual em Saúde (BVS), e nas bases de dados Scientific Eletronic Library Online (SciELO), PubMed e Embase. Com base nos Descritores em Ciências da Saúde (DeCS/MeSH), foram utilizados nas bases selecionadas os descritores em português ketamina e dor aguda, e em inglês ketamine e acute pain. Para os resultados os artigos selecionados para esta pesquisa foram analisados no formato completo, tipo ensaio clínico e agrupados em categorias “uso da ketamina na emergência”, “utilização da ketamina na dor pós-operatória” e “utilização da ketamina nas diversas patologias que apresentam dor aguda”. Para análise crítica e discussão, 15 artigos compuseram os resultados do estudo.
