Oral rehabilitation with All on Four protocol in HIV-infected patient: Case report 2 years follow-up

Resumo: Introduction:Oral rehabilitation with dental implants in people living with HIV (PLwH) is still a topic little studied in the literature, especially due to the health risk due to human immunodeficiency, so the purpose of this work is to report the all-on-four superior oral rehabilitation with guided bone regeneration (GBR) in PVH with 2-year follow-up. Case report:A 58-year-old male patient HIV-infected for 15 years, ART user for 14 years, no previous history of allergies presented to a dental college in northern Brazil with the main complaint of tooth loss, use of temporary removable partial dentures, aesthetic and functional discontent. In the clinical examination, the presence of upper teeth with grade II periodontitis was observed, the patient was systemically healthy and had medical clearance. It was indicated extraction of teeth affected by periodontitis, installation of morse taper implants (12: 3.8x13m/ 22: 3.8x13mm/ 13: 3.8x15mm/ 23: 3.8x15mm), GBR with alogenous bone graft covered by a resorbable collagen membrane to alveolus filling and fabrication of upper protocol prosthesis by the all on four technique.Conclusion: To the authors' knowledge, this is the first case of an all on four with GBRin PLwHin northern Brazil. Through this case, it was observed that dental implants are a viable option for oral rehabilitation when the patient is immunocompetent due to adequate treatment, demonstrating the possibility of longevity of dental implants in this key population.

Caso Clínico: Paciente masculino, de 58 años, con VIH desde hace 15 años y usuario de TARV desde hace 14 años y sin antecedentes de alergias, se presentó en la ciudad de Belém con la principal queja de pérdida de dientes, insatisfacción con el uso de prótesis provisional insatisfacción parcial, estética y funcional removible. En el examen clínico se observó la presencia de dientes superiores con periodontitis grado II, sistémicamente el paciente presenta salud y alta médica satisfactoria. Se indicó extracción de dientes afectados por periodontitis, colocación de implantes de cono morse (12: 3,8x11mm/ 22: 3,8x11mm/ 15: 3,8x13mm/ 25: 3,8x13mm), GBR con injerto óseo xenogénico cubierto por una membrana de colágeno reabsorbible para relleno alveolar y construcción de prótesis mediante la técnica all-on-four. Conclusión: Los autores son conscientes de que este es el primer caso de los cuatro con GBR en PVH en el norte de Brasil. A través de este caso se observó que los implantes dentales son una opción viable para la rehabilitación oral cuando el paciente es inmunocompetente debido a un tratamiento adecuado, demostrando la posibilidad de longevidad de los implantes dentales en esta población clave.Introdução:A reabilitação oral com implantes dentários em pessoas vivendo com HIV (PVH) ainda é um tópico pouco estudo na literatura, especialmente, pelo risco de saúde devido a imunodeficiência humana, logo o intuito desse trabalho é relatar a reabilitação oral all on four superior com regeneração óssea guiada (ROG) em PVH com acompanhamento de 2 anos.Relato de Caso:Paciente sexo masculino, 58 anos, portador do HIV há 15 anos e usuário de TARV durante 14 anos e sem histórico prévio de alergias apresentou-se na cidade de Belém com a queixa principal de perda dentária, insatisfação no uso de prótese parcial removível provisória, descontentamento estético e funcional. No exame clínico observou-se presença de dentes superiores com periodontite grau II, sistemicamente paciente apresentava saúde satisfatória e liberação médica. Foi indicado exodontia dos dentes afetados pela periodontite, instalação de instalação dos implantes cone morse (12: 3.8x11mm/ 22: 3.8x11mm/ 15: 3.8x13mm/ 25: 3.8x13mm), ROG com enxerto ósseo xenógeno recoberto por membrana de colágeno reabsorvível para preenchimento de alvéolo e confecção de prótese protocolo superior pela técnica all on four. Conclusão: É do conhecimento dos autores, que este é o primeiro caso de all on fourcom ROG em PVH no norte do Brasil.
