The new uses of the enamel matrix derivative: literature review

Resumo: Seeking to demonstrate the recent uses of the enamel matrix derivative (EMD), the objective of this work is to develop a literature review of the use of biomaterial in soft tissues and in the treatment of peri-implantitis. The search for scientific articles was carried out at Pubmed, the selected literature was from 2010 to 2020, found with the following descriptors: “Emdogain And Soft Tissue” and “Emdogain And Peri-Implantitis”. Studies on bone regeneration treatment, bone graft, treatment of patients with systemic disease were excluded. The selected sample consisted of 17 articles, and the results found favor the use of EMD. In the healing of soft tissues around teeth and implants, the works report the good hypotheses in relation to cicatricial cellular events (angiogenesis, fibroplasia and reduction of inflammatory markers) and histological evaluations that EMD promotes when facing surgical wounds. In root coverage of different types of recessions, improvements were also seen with the use of EMD, but procedures based on connective tissue grafts still show superior results. In the treatment of peri-implantitis, EMD seems to present a good result as an adjuvant, but more studies need to be performed. The use of EMD is justified by the possibility of achieving periodontal regeneration in the treated sites and by its positive attributes in the healing process of various periodontal procedures in soft tissues, in the techniques of root covering and in the treatment of peri-implantitis.

Se buscaron artículos científicos en Pubmed, la literatura seleccionada fue de 2010 a 2020, se encontró con los siguientes descriptores: “Emdogain And Soft Tissue” y “Emdogain And Peri-Implantitis”. Se excluyeron los estudios sobre el tratamiento de regeneración ósea, el injerto óseo y el tratamiento de pacientes con enfermedades sistémicas. La muestra seleccionada estuvo compuesta por 17 artículos, y los resultados encontrados favorecen el uso de EMD. En la cicatrización de tejidos blandos alrededor de dientes e implantes, los estudios informan las buenas hipótesis con respecto a los eventos de cicatrización celular (angiogénesis, fibroplasia y reducción de marcadores inflamatorios) y evaluaciones histológicas de que el EMD promueve la ganancia de espesor gingival. En el cubrimiento radicular de diferentes tipos de recesiones, también se observaron mejoras con el uso de EMD, pero los procedimientos basados en injertos de tejido conectivo siguen mostrando resultados superiores. En el tratamiento de la periimplantitis, el EMD parece presentar un buen resultado como adyuvante, sin embargo, es necesario realizar más estudios. El uso de EMD se justifica por la posibilidad de lograr una regeneración periodontal en los sitios tratados y por sus atribuciones positivas en el proceso de cicatrización de diversos procedimientos periodontales en tejidos blandos, en técnicas de cobertura radicular y en el tratamiento de periimplantitis.Buscando demonstrar os recentes usos do derivado da matriz de esmalte (EMD), o objetivo deste trabalho é desenvolver uma revisão da literatura do emprego do biomaterial em tecidos moles e no tratamento da periimplantite. Os artigos científicos foram pesquisados no Pubmed, a literatura selecionada foi de 2010 a 2020, encontrada com os seguintes descritores: “Emdogain And Soft Tissue” e “Emdogain And Peri-Implantitis”. Foram excluídos trabalhos sobre tratamento de regeneração óssea, enxerto ósseo, tratamento de pacientes com doença sistêmica. A amostra selecionada consistiu em 17 artigos, e os resultados encontrados favorecem o uso do EMD. Na cicatrização dos tecidos moles ao redor de dentes e implantes, os trabalhos relatam as boas hipóteses em relação aos eventos celulares cicatriciais (angiogênese, fibroplasia e redução de marcadores inflamatórios) e avaliações histológicas que EMD promove ganho de espessura gengival. No recobrimento radicular de diferentes tipos de recessões, as melhorias também foram observadas com o uso do EMD, porém os procedimentos baseados em enxertos de tecido conjuntivo ainda apresentam resultados superiores. No tratamento da periimplantite o EMD parece apresentar um bom resultado como adjuvante, entretanto mais estudos precisam ser realizados.
