Resumo: | Introduction: Brazil registered a significant reduction in the neonatal mortality rate between the periods of 1990 and 2019. However, it still remains with values three times higher than countries such as Canada and the United States. In this sense, intensive care acts as the main support tool for these patients who would die, making it important to know the reasons why newborns are admitted to intensive care, so that it is possible to set goals that reduce the high neonatal mortality still persistent in the country. Objective: To analyze predictors of mortality and admission of newborns in a neonatal intensive care unit. Methods: Observational retrospective cohort study developed through the analysis of medical records of neonates admitted to intensive care at a hospital in southern Brazil, and their respective mothers, from January 1, 2020 to June 31, 2022. Results: Data from 259 neonates and their mothers were evaluated. Respiratory distress was present in 79.2% of neonates, prematurity in 69.9% and low birth weight in 57.5%. Users of positive ventilation had an 80.4% lower risk of death. Death was found in 6.9%. Conclusion: Respiratory distress, prematurity and low birth weight were the main predictors of admission. Extremely premature infants, low weight, congenital diseases, undergoing orotracheal intubation, surgery, resuscitation maneuvers and use of vasoactive drugs were associated with death. A possible association between high values of prematurity and the analysis interval of patients, performed during the COVID-19 pandemic, is questioned. |

Contudo, ainda permanece com valores três vezes maiores que países como Canadá e Estados Unidos. Neste sentido, a terapia intensiva age como principal ferramenta de suporte para estes pacientes que viriam a óbito, tornando-se importante o conhecimento sobre quais motivos levam o recém-nascido a ser admitido em terapia intensiva, para que seja possível traçar metas que reduzam a elevada mortalidade neonatal ainda persistente no país. Objetivo: Analisar fatores preditores de mortalidade e admissão de recém-nascidos em uma unidade de terapia intensiva neonatal. Métodos: Estudo observacional do tipo coorte retrospectiva desenvolvido através da análise de prontuários de neonatos internados em terapia intensiva em um hospital no Sul do Brasil, e suas respectivas progenitoras, no período de 01 de janeiro de 2020 a 31 de junho de 2022. Resultados: Foram avaliados dados de 259 neonatos e suas progenitoras. Desconforto respiratório esteve presente em 79,2% dos neonatos, prematuridade em 69,9% e baixo peso em 57,5%. Usuários de ventilação positiva tiveram 80,4% menos risco de morte. O óbito foi encontrado em 6,9%. Conclusão: Desconforto respiratório, prematuridade e baixo peso foram os principais diagnósticos preditores de admissão. Prematuros extremos, baixo peso, doenças congênitas, submetidos à intubação orotraqueal, cirurgia, manobras de reanimação e uso de fármacos vasoativos foram associadas ao óbito.
