Acellular dermal matrix for root coverage in unitary gingival recession: case report

Resumo: | Introduction: Gingival recession (GR) is defined as the apical migration of the gingival margin, resulting in root exposure in the oral environment, it is known that among its multifactorial etiology the use of orthodontic appliance is cited as a risk factor for GR in patients with a thin periodontal phenotype. Objective: This paper aims to report a case of single root coverage using acellular dermal matrix (MDA) graft. Case Report: Female patient, 24 years old, non-smoker and without systemic alterations, attended the dental clinic of a college in northern Brazil with the main complaint of dentin hypersensitivity in the posterosuperior region. The clinical examination showed stage I type 1 recession in tooth 25 associated with orthodontic movement in the buccolingual direction, the GR of tooth 15 had 2mm (cervico-apical direction) and 1mm (mesiodistal direction), it was also observed Thin scalloped phenotype, no interproximal bone loss, no bleeding on probing and dental biofilm. The indicated treatment was root coverage using the coronal slip technique with MDA fixed with sutures. Conclusion: Given the above, we verified that the clinical results reported in this study indicated a great clinical result with excellent predictability of success, increased keratinized mucosal band and clinical insertion gain. |

Objetivo: Este artículo tiene como objetivo reportar un caso de cobertura radicular única utilizando injerto de matriz dérmica acelular (MDA). Caso clínico: Paciente mujer, 24 años, no fumadora y sin alteraciones sistémicas, acudió a la consulta odontológica de una facultad del norte de Brasil con el principal síntoma de hipersensibilidad dentinaria en la región posterosuperior. El examen clínico mostró recesión en estadio I tipo 1 en el diente 25 asociado a movimiento ortodóncico en dirección vestibulolingual, el GR del diente 15 tenía 2 mm (dirección cérvico-apical) y 1 mm (dirección mesio-distal). Fenotipo festoneado fino, sin hueso interproximal pérdida, sin sangrado al sondaje y biofilm dental. El tratamiento indicado fue la cobertura radicular mediante la técnica de deslizamiento coronal con MDA fijado con suturas. Conclusión: Dado lo anterior, verificamos que los resultados clínicos reportados en este estudio indicaron un gran resultado clínico con excelente predictibilidad de éxito, aumento de la banda mucosa queratinizada y ganancia de inserción clínica.Introdução: A recessão gengival (RG) é definida como a migração apical da margem gengival, resultando na exposição radicular no meio bucal, sabe-se que dentre a sua etiologia multifatorial cita-se o uso de aparelho ortodôntico como um fator de risco para RG em pacientes com fenótipo periodontal delgado. Objetivo: O presente trabalho tem como objetivo relatar um caso de recobrimento radicular unitário utilizando enxerto de matriz dérmica acelular (MDA). Relato de Caso: Paciente sexo feminino, 24 anos, não fumante e sem alterações sistêmicas, compareceu a clínica odontológica de uma faculdade no norte do Brasil com a queixa principal de hipersensibilidade dentinária na região pôstero-superior. No exame clínico verificou-se estágio I recessão tipo 1 no dente 25 associada a movimentação ortodôntica no sentido vestíbulo-lingual, a RG do dente 15 apresentava 2mm (sentido cérvico-apical) e 1mm (sentido mésio-distal), observou-se ainda fenótipo fino festonado, sem perdas ósseas interproximais, ausência de sangramento a sondagem e biofilme dentário. O tratamento indicado foi o recobrimento radicular pela técnica de deslize coronal com MDA fixada com suturas.
