Approach to hypertensive diseases in pregnancy: optimizing treatment and mitigating harm

Resumo: | This article deals with hypertensive diseases in pregnancy, conditions that represent significant clinical challenges and are recognized as one of the main causes of maternal and perinatal morbidity and mortality. Pregnancy is a period of intense physiological transformations and, although it is a natural phase in many women's lives, it can be accompanied by complications, among which hypertensive diseases stand out. These encompass a spectrum of conditions, from gestational hypertension to more serious forms such as pre-eclampsia and eclampsia. The central aim of this literature review is to explore current therapeutic approaches to optimize the treatment of hypertensive disorders in pregnancy and discuss strategies to mitigate the associated damage. To achieve this goal, a systematic search was carried out in recognized scientific databases such as PubMed, Scopus and Web of Science, selecting articles published in recent years and in the English language that addressed the topic in question. The results obtained indicate that, although there are effective treatments available, there are still gaps in the complete understanding of the pathophysiology of hypertensive diseases in pregnancy, which limits the effectiveness of current interventions. However, multidisciplinary strategies, involving a combination of pharmacological and non-pharmacological approaches, as well as patient education and awareness, have shown promise in optimizing treatment and minimizing the associated risks. The review also highlights the continuing need for research and development in the area, aimed at more personalized and effective therapeutic approaches. |

El embarazo es un periodo de intensas transformaciones fisiológicas y, aunque es una fase natural en la vida de muchas mujeres, puede acompañarse de complicaciones, entre las que destacan las enfermedades hipertensivas. Éstas abarcan un espectro de condiciones, desde la hipertensión gestacional hasta formas más graves como la preeclampsia y la eclampsia. El objetivo central de esta revisión bibliográfica es explorar los enfoques terapéuticos actuales para optimizar el tratamiento de los trastornos hipertensivos en el embarazo y debatir estrategias para mitigar los daños asociados. Para lograr este objetivo, se realizó una búsqueda sistemática en bases de datos científicas reconocidas como PubMed, Scopus y Web of Science, seleccionando artículos publicados en los últimos años y en inglés que abordaran el tema en cuestión. Los resultados obtenidos indican que, aunque se dispone de tratamientos eficaces, todavía existen lagunas en el conocimiento completo de la fisiopatología de las enfermedades hipertensivas en el embarazo, lo que limita la eficacia de las intervenciones actuales. Sin embargo, las estrategias multidisciplinares que incluyen una combinación de enfoques farmacológicos y no farmacológicos, así como la educación y concienciación de los pacientes, han demostrado ser prometedoras para optimizar el tratamiento y minimizar los riesgos asociados. La revisión también subraya la necesidad continua de investigación y desarrollo en este ámbito, con el fin de lograr enfoques terapéuticos más personalizados y eficaces.O presente artigo aborda as doenças hipertensivas na gravidez, condições que representam significativos desafios clínicos e são reconhecidas como uma das principais causas de morbidade e mortalidade materna e perinatal. Contextualizando, a gravidez é um período de intensas transformações fisiológicas, e, embora seja uma fase natural na vida de muitas mulheres, pode ser acompanhada de complicações, entre as quais as doenças hipertensivas se destacam. Estas englobam um espectro de condições, desde a hipertensão gestacional até formas mais graves, como a pré-eclâmpsia e eclâmpsia. O objetivo central desta revisão de literatura é explorar as abordagens terapêuticas atuais para otimizar o tratamento das doenças hipertensivas na gravidez e discutir estratégias para mitigar os danos associados. Para alcançar esse objetivo, foi realizada uma busca sistemática em bases de dados científicas reconhecidas, como PubMed, Scopus e Web of Science, selecionando artigos publicados nos últimos anos e em língua inglesa que abordassem o tema em questão. Os resultados obtidos indicam que, embora existam tratamentos eficazes disponíveis, ainda há lacunas no entendimento completo da fisiopatologia das doenças hipertensivas na gravidez, o que limita a eficácia das intervenções atuais. No entanto, estratégias multidisciplinares, envolvendo uma combinação de abordagens farmacológicas e não farmacológicas, bem como a educação e conscientização das pacientes, mostraram-se promissoras na otimização do tratamento e na minimização dos riscos associados.
