Actions to raise awareness and prevent Diabetes Mellitus and Systemic Arterial Hypertension: Relato de experiência

INTRODUCTION: Systemic arterial hypertension and diabetes mellitus are chronic non-communicable diseases responsible for a large number of cases of disability and deaths in Brazil. In this context, taking into account all the impact that these diseases have on the health system, it is important that their control, which goes from prevention to treatment, is effective. Thus, the objective of this experience report was to address the practices carried out together with the hypertensive and diabetic community enrolled in the Unidade de Saúde da Família, and to contribute to the health education of this population on the subject. MATERIALS AND METHODS: A survey was conducted with 35 individuals in a catchment area to analyze the local epidemiological profile and, from then on, to outline health education strategies. RESULTS AND DISCUSSION: the following interventions were developed: (1) health education for knowledge of diseases; (2) encouragement of physical activity; and (3) preparation of an informative booklet with information about hypertension and diabetes, diet, use of medications and tips for organizing them, places to perform physical activity in the neighborhood and useful contacts. CONCLUSION: From the project carried out, it was possible to know the gaps in the place and carry out actions at strategic points, in addition to encouraging the participation of the community in health projects carried out by the Health Unit, which can generate a better quality of life for people, help raise awareness about the prevention of chronic non-communicable diseases and the importance of treatment combined with healthy habits.
