Experiences related to breastfeeding and its early interruption: a qualitative study with nursing mothers

Resumo: Breastfeeding is influenced by several correlated factors, which may favor or disadvantage its maintenance, thus it is necessary to know those factors that lead to such interruption in different contexts. This qualitative, descriptive and analytical study investigates the experience regarding breastfeeding and its early interruption among nursing mothers in a primary healthcare service in Bahia, Brazil. Data was collected by means of semi-structured interviews with nursing mothers who interrupted early the breastfeeding of their children. In all, 14 nursing mothers, aged between 19 to 40 years, were interviewed. After being recorded, the interviews were transcribed for later content analysis and organization into analytical categories. Among the main reasons for interrupting breastfeeding were the myths of “weak milk,” “little milk,” and “dried-up milk.” Breast complications, the women’s return to the workplace and university, as well as the experiences of nursing mothers during breastfeeding. Hence, it appears that the act of breastfeeding involves a series of factors that need to be addressed, overcoming the idea of ??full breastfeeding and without hindrances, so that, with support, these women can recognize the challenges, but especially the gains of breastfeeding. The act of breastfeeding involves complex issues, awakening in women both positive and negative feelings that become entangled with the choice of maintaining exclusive breastfeeding. Thus, health professionals must understand the social and cultural context in which women are inserted, placing them as protagonists in this process.

Por eso, es fundamental conocer los factores que conducen a dicha interrupción en diferentes contextos. Este estudio tuvo como objetivo comprender la experiencia con respecto a la lactancia materna y su interrupción temprana entre nutrices en un servicio de atención básica de salud de un municipio de Bahía (Brasil). Esta es una investigación cualitativa, descriptiva y analítica, realizada a través de entrevistas semiestructuradas, aplicadas a nutrices que interrumpieron tempranamente la lactancia materna de sus hijos. En total, se entrevistó a catorce madres lactantes de entre 19 y 40 años. Las entrevistas fueron grabadas y transcritas para el posterior análisis de los contenidos del discurso y la organización en categorías analíticas. Las principales explicaciones de la descontinuación se relacionaron con los mitos de “leche débil”, “poca leche” y “se secó la leche”, las complicaciones de los senos, el regreso de la mujer al trabajo y los estudios, así como las experiencias de madres lactantes durante el curso de la lactancia materna. Por lo tanto, parece que el acto de amamantar implica una serie de factores que deben abordarse, superando la idea de la lactancia materna completa y sin obstáculos, para que estas mujeres puedan reconocer los desafíos, pero especialmente los logros de la práctica de la lactancia materna. La lactancia materna involucra problemas complejos, que despiertan en las mujeres sentimientos positivos y negativos que se adoptan con la opción de mantener la lactancia materna exclusiva. Así, es importante que los profesionales de la salud entiendan el contexto social y cultural en el que se insertan las mujeres, ubicándolas como protagonistas en este proceso.O curso da amamentação é influenciado por diversos fatores correlacionados, que podem favorecer ou não a manutenção do aleitamento. Sendo assim, é fundamental conhecer os fatores que levam a tal interrupção em diferentes contextos. Este estudo objetivou compreender a vivência em relação ao aleitamento materno e sua interrupção precoce entre nutrizes em um serviço de atenção básica à saúde, de um município na Bahia. Trata-se de uma pesquisa de abordagem qualitativa, descritiva e analítica, realizada por meio de entrevistas semiestruturadas, aplicadas às nutrizes que interromperam precocemente o aleitamento materno dos filhos. Ao todo, foram entrevistadas 14 nutrizes com idade entre 19 e 40 anos. As entrevistas foram gravadas e transcritas, para posterior análise dos conteúdos das falas e organização em categorias analíticas. Entre as principais alegações para a interrupção foram citados mitos de “leite fraco”, “pouco leite” e “leite secou”, as intercorrências mamárias, o retorno da mulher ao âmbito de trabalho e aos estudos, bem como as experiências e vivências das nutrizes durante o curso da amamentação. Assim, constata-se que o ato de amamentar envolve uma série de fatores, que necessitam ser abordados, superando a ideia da amamentação plena e sem dificultadores, para que, apoiadas, essas mulheres possam reconhecer os desafios, mas principalmente os ganhos da prática da amamentação. O ato de amamentar envolve questões complexas, despertando nas mulheres sentimentos tanto positivos quanto negativos que se embricam com a escolha de manutenção do aleitamento materno exclusivo.
