Palliative Care in Hospitalized Children and Adolescents with Cancer: the Role of Occupational Therapy

Resumo: Introduction: Palliative care is characterized by relieving symptoms and maintaining quality of life of patients with terminal illness and their families. Objective: To describe the role of the occupational therapist with children and adolescents with cancer, hospitalized and in exclusive Palliative Care. Method: There was a descriptive study of visits of occupational therapy conducted with this population in a pediatric oncology ward in the interior of São Paulo, from January 2010 to August 2012, by reading the charts and records in a specific form of occupational therapy, seeking to identify the socio-demographic and clinical characteristics, as well as the objectives, resources and outcomes of care. It was performed a quantitative analysis with a complementary qualitative approach. Results: 14 children and adolescents from 1 year and 11 months to 18 years, with different diagnoses, were treated. There was a variation between 1 to 11 visits of occupational therapy, and the patients in the study were hospitalized from 1 to 6 times after the establishment of palliative care. The objectives of occupational therapy were: strengthening relationships; assist in hospitalization coping and in the worsening of disease and death; promote occupational performance and encourage performance skills. These objectives were achieved by using a variety of strategies and resources. Conclusion: The role of Occupational Therapy in Palliative Care helps to maintain the quality of life of children and adolescents, providing their active participation in order to build, together with family and therapist, their daily life and their history, until death.

Objetivo: Describir la actuación del terapeuta ocupacional junto a niños y adolescentes con cáncer, ingresados y en Cuidados Paliativos exclusivos. Método: Se hizo un estudio descriptivo de atendimientos en Terapia Ocupacional realizados con esta población en una enfermería oncopediátrica del interior de São Paulo, entre enero de 2010 y agosto de 2012, a través de la lectura de los prontuarios médicos y registros en informes específicos de la terapia ocupacional, buscando identificar las características sociodemográficas y clínicas, así como los objetivos, recursos utilizados y resultados de los atendimientos. Se ha realizado un análisis cuantitativo con un enfoque cualitativo complementario. Resultados: Han sido atendidos 14 niños y adolescentes entre 1 año y 11 meses hasta 18 años, de diagnósticos diferentes. Hubo una variación entre 1 y 11 atendimientos de Terapia Ocupacional, siendo que los pacientes del estudio han sido ingresados de 1 a 6 veces después del establecimiento de los Cuidados Paliativos. Los objetivos de la Terapia Ocupacional fueron: fortalecer el vínculo, auxiliar en el enfrentamiento al ser ingresado, del agravamiento de la enfermedad y del óbito; favorecer el desarrollo ocupacional y estimular habilidades de desarrollo. Estos objetivos han sido alcanzados a través de la utilización de estrategias y recursos variados. Conclusión: La actuación de la Terapia Ocupacional en Cuidados Paliativos contribuye para el mantenimiento de la calidad de vida en niños y adolescentes, proporcionando su participación activa de manera a construir, junto con la familia y el terapeuta, su cotidiano y su historia, hasta el óbito.  Introdução: Os Cuidados Paliativos caracterizam-se pelo alívio de sintomas e manutenção da qualidade de vida de pacientes com doença incurável e de seus familiares. Objetivo: Descrever a atuação do terapeuta ocupacional junto a crianças e adolescentes com câncer, hospitalizados e em Cuidados Paliativos exclusivos. Método: Fez-se um estudo descritivo dos atendimentos de Terapia Ocupacional realizados com a referida população, em uma enfermaria oncopediátrica do interior de São Paulo, no período de janeiro de 2010 a agosto de 2012, por meio da leitura dos prontuários e registros em formulário específico da Terapia Ocupacional, buscando identificar as características sociodemográficas e clínicas, assim como os objetivos, recursos utilizados e resultados dos atendimentos. Realizou- -se uma análise quantitativa com abordagem qualitativa complementar. Resultados: Foram atendidas 14 crianças e adolescentes, entre 1 ano e 11 meses a 18 anos, de diferentes diagnósticos. Houve variação entre 1 a 11 atendimentos de Terapia Ocupacional, sendo que os pacientes do estudo foram hospitalizados de 1 a 6 vezes após o estabelecimento dos Cuidados Paliativos. Os objetivos da Terapia Ocupacional foram: fortalecer vínculo; auxiliar no enfrentamento da hospitalização, do agravamento da doença e do óbito; favorecer o desempenho ocupacional e estimular habilidades de desempenho.
