Relationship between Dietary Factors and Anthropometric and Gastrointestinal Tract Neoplasms: Investigations Done in Brazil

Resumo: | Introduction: Cancer features itself as an important public health issue both in developed and developing countries. Among cancers which affect the gastrointestinal tract, the most common are: colorectal, stomach, oral cavity and esophagus. Inadequate eating habits, obesity, smoking and sedentarism are identified as important risk factors. Objective: The purpose of this study was to trace the profile of population-based studies performed in Brazil which have investigated the relationship between gastrointestinal neoplasms and nutritional factors, such as anthropometry and food intake. Method: Systematic literature review made through bibliographical searches on computerized data banks and in articles which have investigated the relationship between gastrointestinal neoplasms and nutritional factors in Brazil. Results: 15 articles were selected, seven of which were studies about oral cancer, four about stomach cancer, two about esophagus cancer and two about colorectal cancer. Most part was a cross-sectional study followed by case-control and ecological. Among the studies, 14 evaluated neoplasms as to dietary factors and explored alcoholic beverage consumption. Anthropometric factors were investigated by one study only. The concentration of studies in big urban centers, the poor studies production and the small production of epidemiological designs with analytical power to establish possible nutritional risk factors to the studied neoplasms could be noticed. Conclusions: It is necessary to place the importance of more investigation of this relationship in Brazil, so that more directed actions be established in order to change the gastrointestinal cancer epidemiologic situation in the country. |

Introducción: El cáncer es considerado un importante problema de salud pública tanto en países desarrollados como en los en desarrollo. Entre los tipos de canceres que afectan el tracto gastrointestinal los más incidentes son: de colon y recto, estómago, cavidad oral y esófago. Hábitos alimentarios inadecuados, la obesidad, el tabaquismo y el sedentarismo son citados como importantes factores de riesgo. Objetivo: El objetivo del estudio fue describir el perfil de los estudios de población llevados a cabo en Brasil, los cuales investigaron la relación entre el cáncer del tracto gastrointestinal y factores nutricionales, como antropometría y consumo alimentario. Método: Revisión sistemática de la literatura, llevada a cabo por medio de búsquedas bibliográficas en bases de datos electrónicas, de los artículos que investigaron la relación entre los cánceres del tracto gastrointestinal y los factores nutricionales en Brasil. Resultados: Fueron seleccionados 15 artículos, siendo siete estudios de cáncer oral, cuatro de cáncer de estómago, dos de cáncer de esófago y dos de cáncer de colon y recto. La mayor parte fue llevada a cabo por estudio transversal seguido de caso-control y ecológica. Entre los estudios, 14 evaluaron el cáncer en relación a los factores dietéticos y consumo de alcohol. Los factores antropométricos fueron investigados por un solo estudio. Hubo una concentración de estudios en los grandes centros urbanos, pero todavía hay poca realización de estudios y poca producción de los diseños epidemiológicos analíticos para establecer posibles factores de riesgo nutricionales para los cánceres estudiados. Conclusiones: Por lo tanto, es necesario ubicar la importancia de una mayor exploración de esta relación en Brasil, para que sean establecidas medidas más específicas dirigidas a modificar la situación epidemiológica del cáncer gastrointestinal en el país. Introdução: O câncer caracteriza-se como importante problema de saúde pública tanto em países desenvolvidos como em países em desenvolvimento. Entre os cânceres que acometem o trato gastrointestinal, os mais incidentes são: cólon e reto, estômago, cavidade oral e esôfago. Hábitos alimentares inadequados, obesidade, tabagismo e sedentarismo são apontados como importantes fatores de risco. Objetivo: Traçar o perfil dos estudos populacionais realizados no Brasil que investigaram a relação entre as neoplasias do trato gastrointestinal e fatores nutricionais, como antropometria e consumo alimentar. Método: Revisão sistemática da literatura, realizada por meio de buscas bibliográficas nos bancos de dados informatizados, de artigos que investigaram a relação entre neoplasias do trato gastrointestinal e fatores nutricionais no Brasil. Resultados: Foram selecionados 15 artigos, sendo sete estudos sobre o câncer oral, quatro sobre o câncer de estômago, dois sobre o câncer de esôfago e dois sobre o câncer de cólon e reto. A maioria foi estudo transversal, seguido de caso-controle e ecológico. Entre os estudos, 14 avaliaram as neoplasias em relação aos fatores dietéticos e investigaram o consumo de bebida alcoólica. Os fatores antropométricos foram investigados por apenas um estudo. Verificou-se a concentração dos estudos em grandes centros urbanos, a pouca realização de estudos e a pequena produção de desenhos epidemiológicos com poder analítico para estabelecer possíveis fatores de risco nutricionais para as neoplasias estudadas.
