Patients Quality of Life in a Palliative Care Unit: a Transversal Study

Resumo: Introduction: In palliative care, the major objective of treatment is to improve patients' quality of life. This construct encompasses aspects related to symptoms, functioning, social and physical wellbeing. Objective: Assess health-related quality of life of inpatients of a Palliative Care Unit, and establish relationships with symptom burden. Method: This is a cross-sectional study in which patients filled out the QLQ-C15-PAL questionnaire during December of 2013; medications in use, other treatments and social variables were also registered. Results: 29 patients participated in the study. Analysis using the Mann-Whitney U test revealed worse dyspnea scores associated with lower emotional functioning (p=0.028); lower physical functioning scores, along with higher levels of nausea and vomiting and levels of hyporexia related to worse performance-status values (p=0.047, p=0.038 and p=0.018, respectively). Also, patients who had an escort showed a tendency to have lower scores for quality of life (p=0.035). Conclusion: Assessing quality of life is an important aspect of daily practices in palliative care, and it is important to evaluate the patients' own assessment of their quality of life along with their symptom burden.

Introducción: En los cuidados paliativos, el gran objetivo del tratamiento es mejorar la calidad de vida de los pacientes. Este concepto incluye aspectos relacionados con los sintomas, la funcionalidad, y el bienestar fisico y social. Objetivo: Evaluar la calidad de vida de pacientes ingresados en una unidad de cuidados paliativos, tratando de establecer relaciones con los niveles sintomaticos. Método: Estudio transversal realizado com pacientes ingresados en la unidad de cuidados paliativos que respondieron el cuestionario QLQ-C15-PAL, em diciembre de 2013. Fueron registrados medicamentos actuales, otros cuidados y las variables sociales. Resultados: Participaron 29 pacientes en el estudio. El analisis mediante la prueba U de Mann-Whitney mostro peores puntuaciones de disnea asociada con funcionamiento emocional inferior (p=0,028); puntajes bajos de funcionamiento fisico, los niveles mayores de las nauseas y vomitos, y perdida del apetito asociada con valores peores de performance status (p=0,047, p=0,038 y p=0,018, respectivamente). Ademas, los pacientes con cuidador presentaron una tendencia a menor valores de puntuacion a la calidad de vida (p=0,035). Conclusión: La evaluacion de la calidad de vida es un aspecto importante de la practica diaria en el cuidado paliativo, es mportante evaluar conjuntamente la perspectiva del paciente sobre su propia calidad de vida.Introdução: Em Cuidados Paliativos, o grande objetivo do tratamento e a melhora da qualidade de vida dos pacientes. Esse construto engloba aspectos relacionados a sintomas, funcionalidade, bem-estar físico e social. Objetivo: Avaliar a qualidade de vida de pacientes internados em uma unidade de Cuidados Paliativos, demonstrando possíveis relações com os níveis sintomáticos. Método: Estudo transversal realizado com pacientes internados em uma unidade de cuidados paliativos que responderam ao questionário QLQ-C15-PAL da European Organization for Research and Treatment of Cancer em dezembro de 2013. Foram coletadas as variáveis sociais e registrados medicações e cuidados vigentes no tratamento. Resultados: Participaram do estudo 29 pacientes. A analise pelo teste U de Mann-Whitney evidenciou piores escores de dispneia associados a menor funcionalidade emocional (p=0,028); escores de funcionalidade física menores, níveis maiores de náuseas e vômitos, e de hiporexia associados a piores valores de performance status (p=0,047, p=0,038 e p=0,018, respectivamente). Alem disso, pacientes com acompanhante apresentaram tendencia a menores valores de escores de qualidade de vida (p=0,035).
