Gastric Neuroendocrine Tumors: Case Series

Resumo: Introduction: Gastric neuroendocrine tumors of the stomach, also known as gastric carcinoids, are derived from enterochromaffin cells of the stomach. Despite being a rare neoplasm, they deserve special attention due to their distinct and less aggressive behavior compared to gastric adenocarcinoma. Objective: To analyze in retrospect the cases of gastric neuroendocrine tumors treated at the Abdominal-pelvic Surgery Service of the Brazilian National Cancer Institute; to devise the pre-operation and treatment routines for the gastric carcinoid. Material and Methods: We performed a retrospective analysis of the experience of the Brazilian National Cancer Institute concerning the management of these tumors from 1997 to 2008. Results: Seventeen cases of gastric carcinoid tumors have been reported. After an extensive review of the subject, the new staging as well as the treatment for gastric carcinoid tumor were applied according to the guidelines provided by European Neuroendocrine Tumor Society and to our study. Conclusion: Gastric carcinoid tumors require an individualized treatment in order to avoid unnecessary or very conservative aggressive therapies. The staging and treatment guidelines proposed by the European Neuroendocrine Tumor Society proved reproducible and easy to apply regarding the prognosis, and are currently established as routines of this Service.

A pesar de una neoplasia rara, merece una atención especial debido al comportamiento diferente y menos agresivo que el adenocarcinoma gástrico. Objetivo: Analizar retrospectivamente los casos de tumores neuroendocrinos gástricos tratados en el Servicio de Cirugía Abdominopélvica del Instituto Nacional del Cáncer de Brasil; elaborar la rutina preoperatoria y de tratamiento para el carcinoide gástrico. Material y Métodos: Se realizó un análisis retrospectivo de la experiencia del Instituto Nacional del Cáncer, en el manejo de estos tumores entre los años 1997 y 2008. Resultados: Se reportaron 17 casos de tumores carcinoides gástricos. Después de la revisión extensa del tema, se aplicaron la nueva puesta en escena y directrices de tratamiento del carcinoide gástrico basado en las directrices de European Neuroendocrine Tumor Society y en nuestro estudio. Conclusión: El tumor carcinoide gástrico requiere un tratamiento individualizado a fin de evitar terapias agresivas innecesarias o demasiado conservadoras. La puesta en escena y directrices de tratamiento propuestas por European Neuroendocrine Tumor Society ha mostrado ser de fácil aplicación reproducibles y relacionadas con el pronóstico, siendo establecidas, en la actualidad, como rutinas de ese servicio.    Introdução: Os tumores neuroendócrinos do estômago, também conhecidos como carcinoides gástricos, são provenientes das células enterocromafins do estômago. Apesar de serem uma neoplasia rara, merecem uma atenção especial devido ao comportamento distinto e menos agressivo do que o adenocarcinoma gástrico. Objetivo: Analisar retrospectivamente os casos de tumores neuroendócrinos gástricos tratados no Serviço de Cirurgia Abdominopélvica do Instituto Nacional de Câncer; elaborar a rotina pré-operatória e de tratamento para o carcinoide gástrico. Material e Métodos: Realizou-se análise retrospectiva da experiência do Instituto Nacional do Câncer, no manejo desses tumores, entre os anos de 1997 e 2008. Resultados: Reportaram-se 17 casos de tumores carcinoides gástricos. Após ampla revisão do tema, aplicaram-se o novo estadiamento e as linhas de tratamento do carcinoide gástrico, baseado nos guidelines do European Neuroendocrine Tumor Society e nesta casuística. Conclusão: O tumor carcinoide gástrico necessita de um tratamento individualizado, a fim de se evitar terapias agressivas desnecessárias ou conservadoras em demasia.
