Coverage of Papanicolaou Exams of 25 to 59 year-old Women in Maringá - PR, Brazil

Resumo: Introduction: Cancer is a major challenge faced by the current model of Public Health. Objective: To determine the coverage of the completion of Pap smears in women 25 - 59 years old, ≥ three years or never did, according to sociodemographic variables and network of health service used. Method: A population-based study with analytical cross-sectional design in the city of Maringa, Parana, in the period from October to December 2006. We used a questionnaire administered to 291 household interviews with women through stratified random sampling, which represented the 326 districts of the city and 318,953 inhabitants. For data analysis, we applied the logistic regression model. Results: The coverage of the exam in the last three years was 87.6%, and 12.4% of the women have never done it or did not do it over the last three years. There was no significant association between the socio-demographic data and the conduction of the screening test, using univariate or multivariate. The SUS-network covered 54.3% of tests last year preceding the interview. Failure to perform the test showed significant differences in the lower classes. Conclusion: Considering the goal set by the World Health Organization - 80.0% coverage of Pap smear in the last three years to achieve the program's effectiveness, it can be stated that, in Maringa, coverage is adequate. The test being performed annually was predominant for users of the Unified Health System - SUS , not meeting the recommendations of the Ministry of Healthevery three years.

Objetivo: Determinar la cobertura del Papanicolaou en mujeres de 25-59 años, ≥ que tres años o que nunca lo hicieron, según las variables sociodemográficas y la red de servicio de salud utilizada. Método: Un estudio de base de la población, analítico con diseño transversal realizado en la ciudad de Maringá, Paraná, en el período de octubre a diciembre de 2006. Se utilizó una encuesta aplicada con entrevista domiciliaria a 291 mujeres a través de muestra aleatoria estratificada, lo que representa los 326 barrios de la ciudad y 318.953 habitantes. Para el análisis se aplico el modelo de regresión logística. Resultados: La cobertura de la prueba en los últimos tres años fue de 87,6% mujeres y 12,4% nunca han hecho o lo hicieron a más de tres años. No se encontró ligación significativa de los datos sociodemograficos y la prueba preventiva. El Sistema Único de Salud (SUS) ha cubierto el 54,3% de las pruebas del último año que antecedió a la entrevista. La no realización de la prueba presento diferencias significativas en las clases bajas. Conclusión: Teniendo en cuenta el objetivo esperado por la Organización Mundial de la Salud, el 80,0% de cobertura de la prueba de Papanicolaou en los últimos tres años para lograr la eficacia del programa, se puede afirmar que la cobertura en Maringá está adecuada. La periodicidad anual de realización fue predominante para las usuarias del SUS, no cumpliendo com las recomendaciones del Ministerio de Salud de de ser cada 3 años.  Introdução: O câncer é um dos grandes desafios enfrentados pelo atual modelo de Saúde Pública. Objetivo: Determinar a cobertura do Papanicolaou em mulheres de 25 a 59 anos, igual ou maior que três anos ou que nunca realizaram, segundo variáveis sociodemográficas e rede de serviço de saúde utilizada. Método: Estudo de base populacional analítico com desenho transversal realizado na cidade de Maringá, Paraná, no período de outubro a dezembro de 2006. Utilizou-se questionário aplicado com entrevista domiciliar a 291 mulheres por meio de amostragem aleatória estratificada, as quais representaram os 326 bairros da cidade e 318.953 habitantes. Para análise realizada por regressão logística. Resultados: A cobertura do exame nos últimos três anos foi de 87,6% mulheres e 12,4% nunca realizaram ou fizeram há mais de três anos. Não se observou qualquer associação significativa dos dados sociodemográficos e o exame preventivo. O Sistema Único de Saúde (SUS) cobriu 54,3% dos exames do último ano precedente a entrevista. A não realização do exame apresentou diferenças significativas nas classes menos favorecidas. Conclusão: Considerando-se a meta preconizada pela Organização Mundial da Saúde, de 80,0% de cobertura do exame Papanicolaou nos últimos três anos para se conseguir a efetividade do programa, pode-se afirmar que em Maringá a cobertura está adequada.
