Alterations in the uterine cervix in women taken care of in a Basic Health Unit in Campinas-SP

Resumo: This is a quantitative, retrospective-exploratory study with the aim of identifying the frequency of alterations in the uterine cervix and the profile of the women with altered results in the oncotic colpocytology examination in a Basic Health Unitin Campinas, in São Paulo. Data collection was carried out by an analysis of the medical register of the women who showed alterations in examinations during 2004, using a questionnaire as an instrument to collect data. The information gathered was transferred to a spread sheet of the Epi-info program, for compilation and analysis of descriptive statistics. In 2004, there were 1982 examinations, of which 24.7% showed alterations.433 medical registers were analyzed, where 47.8% of the women were under the age of 30, and the majority (61,5%) initiated their sexual activity before the age of 18;52.9% used hormone contraceptives and only 12.8% reported the use of condoms. The main alterations found were bacterial vaginosis caused by the Gardnerella vaginalis; 52.9%, candidiasis caused by the Candida sp in 15,0% and Cocobacilos in 15,5%. The presence of cervical intraepithelial neoplasm grade I associated with the HPV emerged in 1,8% of medical registers and only 0,7% had NIC in more advanced grades. These data are useful in planning strategies for the primary prevention of local cervical cancer. The elaboration and the promotion of education and surveillance programs in this population is important, in order to reduce morbidity and mortality due to uterine neoplasms.

La recogida de datos se realizó analizando los expedientes de las mujeres que presentaron alteraciones en 2004 con un cuestionario usado como instrumento. Las informaciones obtenidas fueron transferidas a una planilla del programa Epi-info, para compilación y análisis estadístico descriptivo. En 2004 se colectaron 1.982 análisis de los cuales el 24,7% estaba alterado. Se analizaron 433 expedientes: el 47,8% de las mujeres tenía menos de 30 años y la mayoría (61,5%) inició su actividad sexual antes de los 18 años; 52,9% utilizaba anticonceptivos hormonales y sólo un 12,8% usaba preservativos. Las principales alteraciones presentadas fueron la vaginitis bacteriana causada por Gardnerella vaginalis, constituyendo 52,9% de las alteraciones, candidiasis causada por Candida sp en 15,0% y presencia de cocobacilos en 15,5% de los análisis. La presencia de neoplasia intracervical grado I asociada al HVP se observó en 1,8% de los expedientes analizados y solamente un 0,7% presentó NIC en grados más avanzados. Estos datos ayudan a planificar las estrategias para la prevención primaria del cáncer cervical local. Es importante elaborar y fomentar programas de educación y rastreo en esta población para lograr reducción en la morbi-mortalidad femenina causada por neoplasias uterinas.Trata-se de um estudo quantitativo, retrospectivo-exploratório, cujo objetivo é identificar a freqüência de alterações cérvico-uterinas e o perfil das mulheres com resultado alterado no exame de colpocitologia oncótica colhidos em uma Unidade Básica de Saúde em Campinas-SP. As principais alterações apresentadas foram a vaginose bacteriana, causada pela Gardnerella vaginalis, constituindo 52,9% das alterações; a candidíase, causada pela Candida sp em 15,0%;e a presença de cocobacilos em 15,5% dos exames. A presença de neoplasia intracervical grau 1 associado ao HPV emergiu em 1,8% dos prontuários analisados e somente 0,7% apresentaram NIC em graus mais avançados. Esses dados auxiliam no planejamento de estratégias para a prevenção primária do câncer cervical local.
