The Management of Emesis in an Oncology Unit: the Need for Pharmaceutical Intervention in Real Time

Resumo: Introduction: Nausea and vomiting are among the most common effects of this treatment, affecting approximately 70-80% of patients. The antiemetic therapy for chemotherapy aims to prevent these effects. Objective: To verify the conformity of the antiemetic therapy prescribed in an oncology service according to international recommendations. Method: A retrospective descriptive study of the data collection of chemotherapy prescriptions of the oncology service of a university hospital in Belem. The chemotherapy prescriptions from ambulatory patients seen over the period of one year were analyzed. The classification of the Emetogenic level of the chemotherapeutic was in accordance to the recommendations of the Multinational Association of Supportive Care in Cancer, American Society of Clinical Oncology and National Comprehensive Cancer Network, and the classification of the Emetogenic level of chemotherapeutic associations was based on the algorithm proposed by Hesketh and collaborators. Results: Of the 143 prescriptions analyzed, 27.3% presented a low Emetogenic level, 0.7% moderate and 72% high. The study showed variability between the antiemetic prescriptions analyzed and the main guidelines. Regarding the nonconformities identified in the antiemetic prescriptions, 53,8% were identified for low Emetogenic level protocols, involving association or dose; 100% for moderate level, 27,2% for high, both concerning nonconformities of dose. Conclusion: Proper prevention of nausea and vomiting is essential to preserve the cancer patients’ quality of life. Thus, an intra-hospital protocol is required for anti-emetic prescription according to the international guidelines. This research also draws attention to the need for pharmaceutical intervention in real-time in order to contribute to the rational use of medicines.

Introducción: Las nauseas y los vomitos son algunos de los efectos secundarios mas comunes de la quimioterapia, afectan aproximadamente al 70-80 % de los pacientes. El tratamiento antiemetico en la quimioterapia tiene el objetivo de prevenir la aparicion de estos efectos. Objetivo: Verificar la conformidad de las prescripciones de antiemeticos, prescrita en um servicio de oncologia, de acuerdo con las recomendaciones internacionales. Método: Estudio descriptivo retrospectivo de la recogida de datos de las recetas de quimioterapia del servicio de oncologia de un hospital universitario en Belem. Fueron analizadas las recetas de los pacientes ambulatorios reunidas durante el periodo de un ano. Para la clasificacion del potencial emetogeno de la quimioterapia aislada, fue utilizada las clasificaciones adoptadas por la Multinational Association of Suportive Care in Cancer, la American Society of Clinical Oncology y la National Comprehensive Cancer Network, y asociaciones de quimioterapicos se utilizo el algoritmo Hesketh y colaboradores. Resultados: Entre las 143 prescripciones analizadas, 27,3 % tenia bajo potencial emetogeno, 0,7 % moderado y 72 % alto. El estudio demostro que habia variabilidad entre las recetas antiemeticos analizados y las principales directrices. En cuanto a las no-conformidades observadas, 53,8 % ocurrieron en el manejo de la terapia de potencial emetogeno bajo, involucrando asociacion o dosis; 100 % en el nivel moderado y 27,2 % en el nivel alto, en ambos no- onformidades de dosis. Conclusión: La prevencion adecuada de las nauseas y los vomitos es esencial para preservar la calidad de vida de pacientes con cancer. De esta manera se requiere un protocolo intra-hospitalario para la prescripcion antiemeticos de acuerdo con las directrices internacionales. Esta investigacion tambien apunta a la necesidad de una intervencion farmaceutica en tiempo real a fin de contribuir al uso racional de los medicamentos.Introdução: Nauseas e vomitos estao entre os efeitos mais frequentes da quimioterapia antineoplasica, afetando cerca de 70% a 80% dos pacientes. A terapia antiemetica objetiva prevenir o surgimento desses efeitos. Objetivo: Verificar a conformidade das prescricoes de antiemeticos, em uma unidade oncologica, de acordo com as diretrizes internacionais. Método: Estudo retrospectivo-descritivo a partir da coleta de dados das prescricoes de quimioterapia do servico de oncologia de um hospital universitario de Belem. Foram analisadas as prescricoes dos pacientes ambulatoriais atendidos durante o periodo de um ano. Para a classificacao do nivel emetogenico de quimioterapicos isolados, utilizaram-se as classificacoes adotadas pela Multinational Association of Suportive Care in Cancer, American Society of Clinical Oncology e National Comprehensive Cancer Network; e, para associacoes de quimioterapicos, o algoritmo de Hesketh e colaboradores. Resultados: Entre as 143 prescricoes analisadas, 27,3% apresentaram nivel emetogenico baixo, 0,7% moderado e 72% alto. O estudo demonstrou que houve variabilidade entre as prescricoes de antiemeticos analisadas e as principais diretrizes. Quanto as não conformidades observadas, 53,8% ocorreram no manejo da terapia de nivel emetogenico baixo, envolvendo associacao ou dose; 100% no nivel moderado e 27,2% no nivel alto, relacionados a dose. Conclusão: A prevencao adequada das nauseas e vomitos e essencial para preservar a qualidade de vida dos pacientes oncologicos. Dessa forma, e necessario um protocolo intra-hospitalar para prescricao de antiemeticos conforme as diretrizes internacionais.
