Physiotherapy in Main Sexual Dysfunctions After the Treatment of Cervical Cancer: Bibliographic Review

Resumo: An important problem for the public health system is the considerable number of cases of cervical cancer. The treatment of this neoplasm may cause cervical-vaginal and sexual behavior changes. Thus, the objective of this study was to identify the main sexual dysfunctions that may arise after the treatment of cervical cancer and to verify the physiotherapy interventions for them, through a narrative bibliographic review. Scientific articles in English, Portuguese and Spanish were found in the SciELO, PubMed, Lilacs and Cochrane databases, from 2000 to 2010. In the crosscheck of the key words, 109 articles were found; 17 of which were selected for meeting the inclusion criteria. Seven were excluded according to the exclusion criteria, leaving ten articles included. The main sexual dysfunctions resulting from the treatment of cervical cancer identified were: hypoactive desire, anorgasmia, decreased arousal, dyspareunia and vaginismus; and the complications: stenosis and vaginal atrophy, decreased lubrication and sensitivity. The resources most often mentioned were the use of vaginal dilators and digital pressure for the treatment of vaginal stenosis; and functional electrical stimulation, kinesiotherapy of the pelvic floor muscles and manual therapy for anorgasmia, vaginismus and dyspareunia. Therefore, the use of physiotherapy for sexual dysfunctions is an important resource and brings positive results. Nevertheless, more studies are needed so as to demonstrate the benefits of the application of physiotherapy directly in patients with sexual dysfunctions caused by cervical cancer.

El tratamiento de esa neoplasia puede traer alteraciones cervico-vaginales y en el comportamiento sexual. Así, el objetivo de este estudio fue identificar las principales disfunciones sexuales después del tratamiento del cáncer de cuello uterino y verificar las intervenciones de fisioterapia en esas, a través de una revisión bibliográfica narrativa. Fueron encontrados artículos científicos en inglés, portugués y español, en las bases de datos SciELO , PubMed, Lilacs y Cochrane, en el periodo 2000 a 2010. Desde la intersección de las palabra claves fueron encontrados 109 artículos, y entre estos se seleccionaron 17 que cumplían los criterios de inclusión. Siete fueron excluidos de acuerdo con los criterios de exclusión, dejando diez artículos incluidos. Las principales disfunciones sexuales decurrentes del tratamiento del cáncer del cuello uterino identificadas fueron: deseo hipoactivo, anorgasmia, disminución de excitación, dispareunia y vaginismo; y las complicaciones: estenosis y atrofia vaginal, disminución de lubrificación y sensibilidad. Los recursos más citados fueron el uso de dilatadores vaginales y presión digital para el tratamiento de la estenosis vaginal; y la estimulación eléctrica funcionales, la cinesioterapia de los músculos del suelo pélvico y terapia manual para anorgasmia, vaginismo y dispareunia. Se concluye que el papel de la fisioterapia en las disfunciones sexuales es un importante recurso y trae resultados positivos. Sin embargo, se necesitan estudios para demostrar los beneficios de la aplicación de los recursos de terapia física directamente en pacientes con disfunción sexual debido al cáncer del cuello uterino.Um importante problema de saúde publica e o numero considerável de casos de câncer do colo do útero. O tratamento dessa neoplasia pode provocar alterações cervicovaginais e no comportamento sexual. Assim, o objetivo deste estudo foi identificar as principais disfunções sexuais pós-tratamento do câncer do colo uterino e verificar as intervenções da fisioterapia nas mesmas, através de uma revisão bibliográfica narrativa. Foram encontrados artigos científicos nos idiomas inglês, português e espanhol, nas bases de dados SciELO , PubMed, Lilacs e Cochrane, no período de 2000 a 2010. No cruzamento das palavras-chave, foram encontrados 109 artigos; destes, 17 foram selecionados, já que preenchiam os critérios de inclusão. Sete foram excluídos de acordo com os critérios de exclusão, restando dez artigos inclusos. As principais disfunções sexuais decorrentes do tratamento do câncer do colo do útero identificadas foram: desejo hipoativo, anorgasmia, diminuição da excitação, dispareunia e vaginismo; e as complicações: estenose e atrofia vaginal, diminuição da lubrificação e sensibilidade. Os recursos mais citados foram o uso de dilatadores vaginais e digitopressão para o tratamento da estenose vaginal; e a eletroestimulação funcional, a cinesioterapia da musculatura do assoalho pélvico e a terapia manual para anorgasmia, vaginismo e dispareunia. Dessa forma, a atuação da fisioterapia nas disfunções sexuais apresenta-se como um importante recurso e traz resultados positivos.
