Analyzing the Operations of the Hospital Cancer Registry in Three Cities in Northeast Brazil

Resumo: Introduction: A Hospital Cancer Registry (HCR) is indispensable for the identification and monitoring of both morbidity and mortality from cancer at the hospital level, but little is known about the operational conditions of these registries in Brazil, mainly those of public health. Objective: To evaluate how the three HCR of Oncology Care Centers in northeastern Brazil operate, through the structure and process analysis, and how the contextual aspects influence the operations of these registries. Method: It is an exploratory multiple case study, which uses a logical model and a matrix of indicators, through direct observation and interviews with members of the registries and directors of these institutions. Results: The three HCR had several points that have compromised their operations, related to its structure and process. Regarding the structure dimension, all registers shared physical space with other activities. One HCR had exclusive staff and equipment and none had written internal routines, record of the activities or an Advisory Committee installed. The lack of support from state coordination, local coordination, information technology, as well as the lack of financial resources destined to its maintenance were reported. In the process dimension, the checking of inconsistencies in the data collection and data entry were not performed, and the teams did not know the coverage of the registries; there was no publication or use of information from the HCR in the planning of these hospitals. Conclusion: The operation of the HCRs was classified as incipient, showing the need to include the situation of those registries in the discussion of strategies to improve the quality of the information on cancer control.

Objetivo: Evaluar el funcionamiento de los RHC en tres Centros de Atención Oncológica en el noreste de Brasil, a través del análisis de la estructura y proceso y la influencia de los aspectos del contexto en el funcionamiento de estos registros. Método: Se trata de un estudio de casos múltiples, exploratorio que utiliza un modelo lógico y una matriz de indicadores, a través de la observación directa y entrevistas con los miembros de los registros y con los directores de las instituciones. Resultados: Los tres RHC presentaron diversos puntos diversos que comprometían a su funcionamiento, relacionados con la estructura y proceso. En la dimensión estructura los registros funcionaron en espacio físico compartido con otras actividades. Un registro disponía de personal y equipos exclusivos y ninguno contaba con rutinas internas por escritura, registro de las actividades y Comité Asesor. Se reportó la falta de apoyo de los coordinadores estatales, de la coordinación local, del sistema, además de la ausencia de los recursos financieros destinados a su mantenimiento. En la dimensión proceso no se llevó a cabo la verificación de inconsistencias en la recogida y tecleo de los datos no fue llevado a través, los equipos no sabían de la cobertura de los registros y no había ninguna publicación o uso de informaciones de los RHC en el planeamiento de los hospitales. Conclusión: El funcionamiento de la RHC fue incipiente y demostró la necesidad de incluir la situación de estos registros en la discusión de estrategias para la mejoría de la calidad de la información en el control del cáncer.  Introdução: O Registro Hospitalar de Câncer (RHC) é indispensável para identificação e monitoramento da morbimortalidade por câncer em nível hospitalar, mas pouco se conhece das condições de funcionamento desses registros no Brasil, principalmente aqueles da rede pública de saúde. Objetivo: Avaliar o funcionamento dos RHC em três Centros de Atenção Oncológica no nordeste brasileiro, através da análise de estrutura e processo e a influência dos aspectos contextuais no funcionamento desses registros. Método: Trata-se de um estudo de casos múltiplos, exploratório, utilizando-se um modelo lógico e uma matriz de indicadores, através da observação direta e entrevistas com os integrantes dos registros e com os diretores dessas instituições. Resultados: Os três RHC apresentavam diversos pontos que comprometiam seu funcionamento, relacionados à estrutura e ao processo. Na dimensão estrutura, os registros funcionavam em espaço físico compartilhado com outras atividades. Um registro dispunha de funcionários e equipamentos exclusivos e nenhum tinha rotinas internas escritas, registro das atividades e Comissão Assessora instalada. Foram relatadas a falta de apoio das coordenações estaduais, da coordenação local, da informática, além da ausência de recursos financeiros destinados à sua manutenção. Na dimensão processo, a verificação de inconsistências na coleta e digitação dos dados não era realizada, as equipes não conheciam a cobertura dos registros e não havia publicação ou utilização das informações dos RHC no planejamento dos hospitais.
