Introduction of Early Diet on Postoperative of Colorectal Cancer Surgery: Developing a Protocol for a Diet

Resumo: Introduction: The early feeding after surgery can speed healing, protecting the anastomosis complications, reducing the length of hospitalization. Objective: To evaluate the early introduction of an oral diet, through a protocol of diet evolution in patients who underwent surgery for colorectal cancer. Method: Retrospective descriptive study, using records of patients who underwent elective surgery, and: diagnosis, height, weight, length of hospitalization, start date of diet, diet consistency, surgical complications and symptoms were evaluated. They were divided into two groups: early onset and late diet protocol without protocol. Results: 124 patients included, 70 women and 54 men. The rate of acceptance of the protocol was 96%.The group that followed the protocol was 100 and 24 patients did not follow it. The group that did not follow the protocol showed a higher frequency of gastrointestinal symptoms such as bloating (3.22%) and constipation (4.03%), symptoms that were not present in the other group. Surgical complications had a higher incidence in the group that did not follow the protocol, showing a 4.1 times greater chance of developing surgical complications. Patients with the early diet evacuated, on average, on the 5th day after surgery, with an average lower than when that compared to patients who did not follow the protocol, who evacuated on the 9th postoperative day. The length of stay was lower in the group with a protocol, that comprised 3.7 days. Conclusion: Following the diet protocol suggested a decreased rate of surgical complications, gastrointestinal symptoms presented and reduced the time of admission.

Objetivo: Evaluar la introducción precoz de la dieta oral, a traves de un protocolo de evolución de la dieta en pacientes sometidos a cirugía del el cáncer colorrectal. Método: Estudio descriptivo retrospectivo, utilizando los registros de los pacientes sometidos a cirugía electiva han sido estudiados: el diagnóstico, talla, peso, dias de hospitalización, comienzo de la dieta, la consistencia de la dieta, complicaciones quirúrgicas y los síntomas presentados. Se dividieron en dos grupos: precoz con protocolo de dieta y tardio sin protocolo. Resultados: 124 pacientes incluidos, 70 mujeres y 54 Hombres. La tasa de aceptación del protocolo fue del 96%. El grupo que siguió el protocolo fue de 100 pacientes y 24 no lo seguieron de síntomas gastrointestinales, como distensión abdominal (3,22%) y estrenimiento (4,03%), síntomas que no estaban presentes en el otro grupo. Las complicaciones quirúrgicas tuvieron una mayor incidencia en el grupo que no siguió el protocolo, presentando la probabilidad de 4,1 veces mayor de desarrollar complicaciones quirúrgicas. Los pacientes con dieta precoz han evacuado, en promedio, el 5° día del postoperatorio, con un tiempo promedio abajo si comparado al grupo de pacientes que no han seguido el protocolo que han evacuado en el 9° día del postoperatorio. El tiempo de hospitalización fue inferior en el grupo com protocolo, de 3,7 días. Conclusión: El seguimiento del protocolo de dieta ha sugerido una disminuición de las tasas de complicaciones quirúrgicas, sintomas digestivos presentados y ha reducido el tiempo de hospitalización.  Introdução: A alimentação precoce no pós-operatório pode acelerar a cicatrização, protegendo a anastomose de complicações, reduzindo o tempo de internamento. Objetivo: Avaliar a introdução precoce de dieta oral, por meio de um protocolo de evolução de dieta, em pacientes submetidos a cirurgia por câncer colorretal. Método: Estudo retrospectivo descritivo, utilizando-se prontuários de pacientes submetidos a cirurgias eletivas, sendo avaliados: diagnóstico, estatura, peso, dias de internamento, inicio da dieta, consistência de dieta, complicações cirúrgicas e sintomas apresentados. Foram divididos em dois grupos: precoce com protocolo de dieta e tardio sem protocolo. Resultados: 124 pacientes incluidos, 70 do sexo feminino e 54 do sexo masculino. A taxa de aceitação do protocolo foi de 96%. O grupo que seguiu protocolo foi de 100 pacientes e 24 não seguiram. O grupo que não seguiu apresentou maior frequência de sintomas gastrointestinais, como distensão abdominal (3,22%) e obstipação (4,03%), sintomas que não estiveram presentes no outro grupo. As complicações cirúrgicas obtiveram maior incidência no grupo que não seguiu o protocolo apresentando uma chance de 4,1 vezes maior de desenvolvimento de complicações cirúrgicas. Os pacientes com dieta precoce evacuaram, em média, no 5° dia do pós-operatório, com tempo médio inferior quando comparado ao grupo de pacientes que não seguiram o protocolo que evacuaram no 9° dia do pós-operatório. O tempo de internamento foi menor no grupo com protocolo, sendo de 3,7 dias.
